OBJECTIVESporadic unilateral vestibular schwannomas are rare in the pediatric population. Little has been reported in the literature on the presentation, tumor size, response to surgical treatment, and recurrence rates in these younger patients. The authors’ goal was to describe their institutional experience with pediatric sporadic vestibular schwannomas and to conduct a meta-analysis of the existing literature to provide further insight into the presentation, tumor characteristics, and surgical outcomes for these rare tumors to help direct future treatment strategies.METHODSThe authors performed a retrospective review of all patients 21 years of age or younger with unilateral vestibular schwannomas and without neurofibromatosis type 2 who underwent resection by the senior authors between 1997 and 2019. A systematic review of the literature and meta-analysis was also performed by entering the search terms “pediatric” and “vestibular schwannoma” or “acoustic neuroma,” as well as “sporadic” into PubMed. Presentation, treatment, clinical outcomes, and follow-up were analyzed.RESULTSFifteen patients were identified at the authors’ institution, ranging in age from 12 to 21 years (mean 16.5 years). Common presenting symptoms included hearing loss (87%), headache (40%), vertigo (33%), ataxia (33%), and tinnitus (33%). At the time of surgery, the mean tumor size was 3.4 cm, with four 1-cm tumors. Four patients had residual tumor following their first surgery, 3 (75%) of whom had significant radiographic regrowth that required further treatment. The literature review identified an additional 81 patients from 26 studies with patient-specific clinical data available for analysis. This resulted in a total of 96 reported patients with an overall average age at diagnosis of 12.1 years (range 6–21 years) and an average tumor size of 4.1 cm.CONCLUSIONSPediatric vestibular schwannomas present similarly to those in adults, although symptoms of mass effect are more common, as these tumors tend to be larger at diagnosis. Some children are found to have small tumors and can be successfully treated surgically. Residual tumors in pediatric patients were found to have a higher rate of regrowth than those in their adult counterparts.

RESUMO -O neurinoma do acústico é o tumor da região ângulo ponto-cerebelar mais comum em indivíduos adultos, sendo raro em crianças sem neurofibromatose. A literatura consultada mostra 18 casos bem documentados de crianças portadoras de neurinomas do VII nervo craniano com idade inferior a 16 anos. Relatamos dois casos de crianças com idade de 9 e 15 anos, portadoras desta patologia e submetidas a tratamento cirúrgico. Nestes casos, assim como nos relatados na literatura, o quadro clínico é semelhante ao do adulto, em que o primeiro sintoma é a surdez progressiva, mais tardiamente seguindo-se sinais e sintomas relacionados à lesão expansiva na fossa posterior.PALAVRAS-CHAVE: nervo vestíbulo-coclear, neurinoma do acústico, tumor pediátrico. Acoustic schwannoma in children without neurofibromatosis: report of two casesABSTRACT -Acoustic schwannoma is the most common tumor of the cerebellopontine angle in adults and is rarely found in children without neurofibromatosis. In the literature there are 18 children under the age of 16 with such tumor. Two female patients with age of 9 and 15 years old with acoustic schwannoma without neurofibromatosis are related. Progressive deafness followed by signs and symptoms of a posterior fossa tumor were the initial complaint of both, as well as of the other related cases and in adulthood.KEY WORDS: acoustic nerve, vestibular schwannoma, pediatric tumor.Os neurinomas do acústico são os tumores que mais frequentemente que acometem a região do ângulo ponto-cerebelar (APC) em indivíduos adultos, correspondendo a cerca de 6 a 8% de todos os tumores intracranianos primários 1 . Sua ocorrência em crianças é rara, havendo somente 18 casos com idade inferior a 16 anos bem documentados e relatados na literatura. Neste estudo apresentamos dois casos de crianças com idade inferior a 15 anos, portadoras de neurinoma do nervo acústico sem história familiar ou evidência clínica de neurofibromatose. RELATO DOS CASOSCaso 1 -SAG. sexo feminino, 15 anos, atendida com história de 3 anos de redução da acuidade auditiva à esquerda (E) passando a apresentar anacusia 8 meses antes de seu primeiro atendimento. Não apresentava antecedentes familiares de neurofibromatose. Ao exame neurológico apresentava anacusia à E e hipoestesia tátil dos três territórios de inervação do V nervo craniano à E. Não foram encontradas lesões de pele sugestivas de neurofibromatose.
