Unexpected Diagnosis of Both Adenocarcinoma of the Colon and Metastatic Lobular Carcinoma of the Breast in the Gastrointestinal Tract

Abstract: Breast cancer rarely metastasises to the gastrointestinal tract. Lobular carcinoma more commonly metastasises to the uterus and appendages, peritoneum, and gastrointestinal tract than other types of breast cancer, while ductal carcinoma has a propensity to metastasise to the lungs, liver, and brain. We describe the case of a patient with no known history of breast cancer, whose primary presentation of lobular breast cancer was with malignant small intestinal and colonic strictures, with coexisting previously u… Show more

“…Breast cancer had not been previously diagnosed. The authors point out that this is the only case in the available literature of coexistence of colon adenocarcinoma and metastatic lobular breast cancer in the same part of the colon, where breast cancer had not been diagnosed previously [14]. The authors described the presence of one tumor next to another (coexistence) and did not describe the presence of metastatic deposits of lobular carcinoma of the breast in the primary adenocarcinoma of the colon.…”

“…Miller et al [14] described the case of a patient with a synchronously diagnosed adenocarcinoma of the colon and metastatic lobular carcinoma in the colon and small intestine. Colon adenocarcinoma was localized in the sigmoid colon.…”

“…GI tract metastases usually occur four to five years after primary breast cancer diagnosis but sometimes 20 or even 30 years later. Occasionally, their presentation is concurrent but, rarely, they may be the first clinical manifestation of breast cancer [ 4 , 19 ]. The rate of GI tract involvement in metastatic breast cancer is around 1% and is found in up to 12% of autopsies [ 18 ].…”

“…The reason for this different metastatic pattern has been suggested to be the loss of E-cadherin [ 18 ]. ILC metastasizes more commonly to the GI tract than NST carcinoma of the breast with an incidence of 4.5% and 0.2%, respectively [ 18 ] and is usually associated with disseminated disease [ 4 ].…”
