Undercarboxylated Osteocalcin Measured with a Specific Immunoassay Predicts Hip Fracture in Elderly Women: The EPIDOS Study1

Abstract: Increased levels of circulating undercarboxylated osteocalcin (ucOC), measured indirectly with the hydroxyapatite (HAP) binding assay, have been shown to predict hip fracture risk in a small group of elderly institutionalized women. The aim of this study was to confirm these findings in a prospective cohort study (EPIDOS prospective study) of 7598 healthy, independently living women over 75 yr of age. One hundred and four women who sustained a hip fracture during a 22-month follow-up period were age matched wi… Show more

“…Future research may benefit from focusing on the various types of OC to ascertain whether there is a relationship present. However, there has been difficulty in measuring ucOC and cOC as few assays exist and it is unclear which assay system provides the most accurate measurements due to problems with comparability and heterogeneity of OC (14, 6668). OC also displays a circadian rhythmicity with a nocturnal peak and thus timing of blood sampling may also contribute to variations in results (69, 70).…”

“…Since a part of the OC enters the bloodstream, the serum OC concentration is used as a marker of bone forma-tion (16,17 (18)(19)(20)(21) and that a high serum concentration of ucOC is related to the risk of fracture in elderly women or osteoporotic patients (22)(23)(24)(25)(26) Another striking finding in this study is that serum concentrations of OC and ucOC follow a very similar time course (27). The precise turnover of OC and ucOC in human serum has not been reported in the literature, although a study in rat models reported that one-half of the administered bone Gla protein may be cleared from serum in less than 5 minutes (17).…”

“…Several clinical studies illustrated the fracture predictive role of bone turnover markers (BTMs) in postmenopausal women independent of bone mineral density (BMD) [32,33]. Increased bone turnover alters bone quality through altering bone microarchitecture.…”
