Seven cases of adrenal myelolipoma comprising 5.8% of total adrenal tumors are described with a male-to-female ratio of 1.3:1. Five were symptomatic, of which 4 had a palpable abdominal mass, 2 cases detected incidentally were associated with carcinoma of the uterine cervix and renal cell carcinoma, respectively. The average age and duration of symptoms at presentation were 56 years (range 38–70 years) and 3.16 months (range 1–9 months). A CT scan was done in all cases, of which 5 showed a nonenhancing mass lesion with fat density diagnosed as adrenal myelolipoma. However, in 1 case radiological diagnosis of liposarcoma was maintained because of the huge size of the lesion whereas in another case the lesion was missed because of associated renal cell carcinoma. Interestingly the right adrenal was involved in all cases and the weight varied from 7 to 2,000 g. No recurrence was noted in the follow-up period (ranging from 3 months to 10 years).

We report the helical computed tomographic findings of a symptomatic giant (11 cm) adrenal myelolipoma with spontaneous extracapsular hemorrhage. Narrow collimation (3 mm) imaging and three-dimensional reconstructions were useful to fully characterize the components and location of the lesion. A literature search showed a striking male and right-sided predominance for spontaneously hemorrhagic myelolipomas.

A total of 44 live cheetahs were examined sonographically between 1989 and 1994. Seventeen of these animals originated from 4 European facilities, the other 27 cheetahs were examined in 3 facilities in Namibia. Evaluation of the cheetahs included a clinical examination, abdominal ultrasound, and complete blood work. Splenic myelolipomas were diagnosed sonographically in 11 cheetahs. In 7 cheetahs the diagnosis was confirmed at necropsy. Nine of the 14 cheetahs in Europe had myelolipomas in the spleen, while in the African cheetahs there were only 2 animals with myelolipomas. Histologically these lesions were characterized by fatty nodules. A more fitting term of Nodular Lipomatosis of the spleen is therefore suggested. All cheetahs with nodular lipomatosis of the spleen, had concurrent chronic diseases at necropsy. An indicator function of splenic nodular lipomatosis for chronic disease and/or stress is possible.

Abstract: We report 2 cases of adrenal myelolipoma with sonographic and computerized tomographic findings. Characteristic radiologic signs of this rare clinical entity must be known to avoid unnecessary surgical procedures.
