Introduction: Gonial angle measurements serve as an important parameter of the craniofacial complex and are usually studied on lateral cephalograms, but recently panoramic radiographs have also been used. The aim of this study is to measure the gonial angle using orthopantomogram and lateral cephalogram based on age, gender, ethnicity, and skeletal malocclusion in a tertiary care hospital. Methods: This is a descriptive cross-sectional study comprising 350 selected radiographs of apparently healthy individuals collected for a period of one year, from the Department of Oral Radiology of a tertiary care centre of Nepal. Gonial angle measurements were recorded from digital panoramic radiograph (both right and left side) and lateral cephalogram. Data was collected in terms of age, gender, ethnicity, and skeletal malocclusion and then statistically analysed using the Statistical Package for the Social Sciences version 20. Results: The mean gonial angle for panoramic right, left and lateral cephalogram was 122.490±7.570, 123.620±7.060, and 124.150±6.910, respectively. There was a decrease in the mean values of the gonial angle observed as age advances seen in all the radiographs. The gonial angle measured in Class III malocclusion was higher in all the radiographs, followed by Class II and Class I. Gonial angles obtained in females were higher than the males in all the radiographs. Finally, the Aryan population showed a higher gonial angle compared to the Mongolian population in all the radiographs. Conclusions: Panoramic radiograph (left side), could be considered as a reliable tool to measure the gonial angle.

This case report presents an alternative approach for the removal of a fractured endodontic instrument from the apical third of the root canal. A 52-years-old female patient was referred for specialized endodontic treatment of the maxillary left first molar due to the presence of a periapical lesion and root canal calcification. After clinical and radiographic examination, the calcification was confirmed. During biomechanical preparation, the fracture of a size 25/.06 NiTi reciprocating file (ProDesign R) occurred in the apical third of the palatine canal. The patient was informed about the complication and agreed with the attempt to remove the fragment. Firstly, a slight wear of the dentinal walls around the fragment was made using ultrasonic inserts under magnification. Then, a customized extractor was made using a hypodermic needle and a handling file whose mechanism is similar to the removal method previously described by Masserann. For this, the hypodermic needle was inserted in the root canal and the handling file was adapted in the needle lumen, with the function of fixing the fragment and removing it from the canal through the opposite movement performed by the fractured file. After associating the technique with a hypodermic needle and using ultrasound and an operating microscope to perform the procedure, the fragment was successfully removed. The association of techniques to remove fragments using only items already present in the endodontic arsenal can eliminate the need for surgical treatment and improve the prognosis of endodontic treatment through a safe, simple and cost-effective method that can be performed in the endodontic clinical routine.

A fratura acidental de um instrumento durante o tratamento endodôntico pode ocorrer devido à falta de conhecimento anatômico do elemento dentário, ao uso de instrumentos inadequados ou à falha técnica do operador. O objetivo do presente estudo foi apresentar um relato de caso clínico da remoção de um instrumento fraturado no terço apical de um pré-molar superior. O fragmento localizava-se no terço apical da raiz vestibular. A primeira tentativa de remoção do fragmento foi realizada sem sucesso. Após 15 dias, em uma nova tentativa, o instrumento foi removido com o auxílio de ultrassom e inser to específico. Entre as sessões, utilizou-se medicação intracanal à base de hidróxido de cálcio, paramonoclorofenol canforado e propilenoglicol. Decorridos 15 dias, o dente mostrou-se assintomático e foi obturado pela técnica da condensação lateral. Assim, a paciente foi encaminhada para a realização da restauração definitiva. Após 3 meses, foi observada radiograficamente a regressão da lesão perirradicular. Conclui-se que a retirada do fragmento no caso descrito se mostrou uma opção eficiente, propiciando a manutenção do dente com ausência de sinais e sintomas.
