Introdução: os pré-molares inferiores representam um desafio no tratamento endodôntico, devido às suas variações anatômicas. Assim, emprega-se a tomografia computadorizada de feixe cônico (TCFC) para o estudo detalhado da anatomia dentária. Objetivo: avaliar, por meio da TCFC, a anatomia dentária de pré-molares inferiores. Métodos: a amostra incluiu imagens de TCFC, nos cortes coronal, sagital e axial, de 200 pré-molares inferiores. O número de raízes, canais e a configuração, de acordo com Vertucci, foram analisados. As variáveis foram associadas e comparadas, a um nível de significância de 5%. Resultados: a configuração do tipo I predominou em ambos os sexos, masculino e feminino, e nos primeiros e segundos pré-molares. Contudo, o teste exato de Fisher apontou que não foi possível confirmar uma associação entre os sexos e a conformação dos canais de acordo com a classificação de Vertucci. Por meio do teste Mann-Whitney, houve evidências amostrais de que o número de raízes (p = 0,04) e de canais (p = 0,01) difere entre os sexos. Em 62% dos primeiros pré-molares e 92% dos segundos pré-molares, foi observada a presença de uma raiz e um canal. O teste de Friedman indicou que o número de raízes e canais também diferiu entre os tipos de dentes (p = 0,02). Conclusões: a TCFC possibilitou a análise, de maneira eficaz e não invasiva, da morfologia dentária, verificando-se que o tipo I foi o mais frequente em ambos os sexos e nos primeiros e segundos pré-molares. Foi possível verificar que o número de raízes e canais difere significativamente tanto entre os sexos quanto entre os tipos de dentes.
A revascularização pulpar é uma terapia indicada para dentes imaturos com necrose pulpar e possibilita a continuidade da formação radicular e o aumento da espessura das paredes dentinárias. O objetivo deste trabalho foi realizar uma revisão de literatura dos diferentes protocolos terapêuticos de revascularização pulpar propostos. Realizou-se uma busca avançada, entre os anos de 2000 e 2017, nas bases de dados eletrônicas PubMed e LILACS, utilizando-se termos indexados no “Medical Subject Heading Terms” (Mesh Terms –MeSH) e “Descritores em Ciências da Saúde” (DeCs). Dentre a literatura obtida, foram selecionados 42 trabalhos que melhor corresponderam aos objetivos propostos.Além disso, incluiu-se 3 artigos pioneiros no estudo da revasculização pulpar. Na literatura é descrito que o sucesso desta terapia está vinculado à adequada descontaminação do canal radicular. Como soluções irrigadoras recomenda-se clorexidina ou hipoclorito de sódio, em conjunto com o EDTA, para posterior aplicação da medicação intracanal (MIC). Como MIC, tem sido proposta a pasta tripla antibiótica (metronidazol, minociclina e ciprofloxacina), contudo, como a presença da minociclina pode causar escurecimento dentário, alguns estudos indicam uma pasta de hidróxido de cálcio. O tempo de permanência da MIC e o período ideal de proservação ainda não foram determinados na literatura. Conclui-se que, apesar de constatados resultados satisfatórios quanto a formação radicular e aumento da espessura dentinária, ainda não há um protocolo ideal definido, o que demanda mais pesquisas sobre o assunto.
A fratura acidental de um instrumento durante o tratamento endodôntico pode ocorrer devido à falta de conhecimento anatômico do elemento dentário, ao uso de instrumentos inadequados ou à falha técnica do operador. O objetivo do presente estudo foi apresentar um relato de caso clínico da remoção de um instrumento fraturado no terço apical de um pré-molar superior. O fragmento localizava-se no terço apical da raiz vestibular. A primeira tentativa de remoção do fragmento foi realizada sem sucesso. Após 15 dias, em uma nova tentativa, o instrumento foi removido com o auxílio de ultrassom e inser to específico. Entre as sessões, utilizou-se medicação intracanal à base de hidróxido de cálcio, paramonoclorofenol canforado e propilenoglicol. Decorridos 15 dias, o dente mostrou-se assintomático e foi obturado pela técnica da condensação lateral. Assim, a paciente foi encaminhada para a realização da restauração definitiva. Após 3 meses, foi observada radiograficamente a regressão da lesão perirradicular. Conclui-se que a retirada do fragmento no caso descrito se mostrou uma opção eficiente, propiciando a manutenção do dente com ausência de sinais e sintomas.
The aim of this study was to evaluate the anesthetic efficacy of the 4% articaine by the buccal infiltration technique (BI) after inferior alveolar nerve block (IANB) with 2% mepivacaine, both with 1:100,000 epinephrine, in the urgency treatment of mandibular molars with symptomatic irreversible pulpitis (SIP). This was a prospective study, with a randomized, blinded, controlled parallel clinical trial experimental design. Thirty patients diagnosed with SIP were randomly assigned, and the control group (G1) received the IANB with 1.8 mL of mepivacaine while the experimental group (G2) also received a supplemental BI with 1.8 mL of articaine. To evaluate the anesthetic efficacy, in addition to lip numbness test, the electric pulp testing (EPT) was used. Additionally, a visual analogue scale verified the presence of pain. Two-way ANOVA and Chi-square tests were applied at a significance level of 5%. There was a reduction in pain perception after the two techniques (p<0.001), with no difference between groups (p=0.479). The anesthetic efficacy was 26.6% for IANB (G1) and 40% for IANB plus BI (G2), without significant difference between the two treatments (p=0.699). The BI with 4% articaine after IANB with 2% mepivacaine, both with 1:100,000 epinephrine, did not significantly increase anesthetic success in patients with SIP.
