Abstract:Certain methods of homicide used by the Italian Mafia are intended to have an admonitory significance. One such method is the so-called "incaprettamento." This study analyzes 18 cases of homicidal ligature strangulation in which the body was found in this typical position. The circumstances of the crime and the macroscopic and microscopic evidence were evaluated to determine whether or not the ligatures on the wrists and ankles were placed antemortem or postmortem.
“…2 A 28-year-old woman bound with chains, lying in the autopsy room. The pattern of the chains is clearly visible, showing similarity to a typical homicide ritual of the Italian mafia called "incaprettamento" [12] Fig. 3 A 48-year-old woman hanging fully dressed in a basement doorway with protective padding of the neck; the feet are tied up recognised, or if they are misdiagnosed at the scene of death or post-mortem examination.…”
Four previously unpublished cases of female asphyxiophilia are presented. All women were found immobilised by obviously self-tied ropes, string or handcuffs. The women, who were alone at the time of death, died of a lethal paraphilia. The autopsies revealed asphyxiation as the cause of death, caused in two cases by suffocation as a result of hanging and strangulation and in the other two cases by plastic bags placed over the individuals head. In one case there was additional evidence at the scene that the deceased had inhaled ether. In none of the four cases was there any indication that the asphyxiation was due to homicide or suicide. Thus they can be described as accidental autoerotic deaths (AAD). The four cases closely mirror findings from scenes of male AADs, although autoerotic practices are generally believed to be rarer among females than in males.

“…Often, the binding of the body is done postmortem, after the victim is strangled with the same rope later used in binding. 15 Although similar methods could be determined in the literature, none of them were identical to the tying style we confronted. So we could not determine and connect any torture methods with the tying method we found in these cases.…”
This study aims at describing autopsy findings of 19 victims killed by various torture methods applied by a terror organization after they had been kidnapped by this organization, as well as the unusual and unique binding style applied during these killings. The present study also aims at describing the unusual killing method of binding for literature and discussing international descriptions of torture.

“…4 Incaprettamento can be regarded as a rare variant of ligature strangulation, common in North America and Europe and frequently involving female victims in the context of rape homicide involving positional asphyxia because the body position can interfere with the normal pump function of respiration. 4 Incaprettamento can be regarded as a rare variant of ligature strangulation, common in North America and Europe and frequently involving female victims in the context of rape homicide involving positional asphyxia because the body position can interfere with the normal pump function of respiration.…”
Incaprettamento is a ritual strangulation that represents a method of homicide typical of the Italian Mafia. While the victim is in the prone position, he/she is bound by one end of a rope, creating a slipknot around the throat, while the other end is used to tie the limbs behind the back. Forensic investigations reveal that in most cases, the binding of the extremities and the positioning of the victim are carried out after death due to others means and are intended to hold somebody in contempt and to punish betrayers. When the victim is tied while alive, the death is caused by self-strangulation because it is impossible to maintain the legs in this forced position. Here, we describe in detail a case with a lot of similarities to incaprettamento, although we define it as atypical because of the unusual methods of the actual binding and the circumstances in which death occurred. In fact, the investigation of the crime scene, the external and internal findings, and the histologic examination result show that this is a crime of passion.
