Two cases of acute macular neuroretinopathy

Abstract: Vasoconstriction in the chorioretina may be pathogenic in AMNR. Functional complaints precede fundus lesions in AMNR. And, IR light is superior to red-free or white light imaging in detecting typical fundus lesions in AMNR both early and late in the course of the disease.

“…It occurs predominantly in young women, often preceded by an episode of flu, which matches our patients [2][3][4][5][6]. Other associations, such as oral contraceptives, epinefrin, extensive coffee consumption, and hypotensive episodes have raised the question of vascular involvement [2,4]. Even though the pattern of perifoveal vasculature has been proposed to be responsible for the lesion morphology in this context, the exact mechanism remains unclear [6].…”

“…Interestingly one case (case 3) presented with superficial haemorrhage as well as with characteristic AMN lesions, which corresponded to hyperautofluorescence using the 488 nm FAF filter (and hypoautofluorescence at 787 nm). This is not characteristic for AMN, as most previous reports have described either no visible FAF changes, subtle hypoautofluorescent changes on 488 nm FAF or subtle hyperautofluorescent changes on 787 nm FAF 2 16. However one previous report detected AMN lesions presenting with subtle hyperautofluorescence on 488 nm FAF similar to our case 17.…”

“…All these features have been previously described in patients with acute macular neuroretinopathy (AMN), 25 an uncommon clinical condition. A number of reports have also described outer retinal abnormalities in AMN, 3,4,6 based on OCT findings.…”
