After anterior cruciate ligament reconstruction (ACLR), there is a higher re‐injury rate to the contralateral limb in athletes who undergo surgery using a bone‐patellar‐tendon‐bone (BPTB) autograft than using a semitendinosus and gracilis hamstring tendon (HT) autograft. This may be influenced by differing lower‐limb loading asymmetries present when athletes of each graft type return to play (RTP). The aim of this study was to compare bilateral countermovement jump (CMJ) phase‐specific impulse asymmetries between athletes with BPTB and HT autografts 9 months post‐ACLR, and to identify the relationship between impulse and isokinetic strength asymmetries. Male field sport athletes with a BPTB (n = 22) or HT (n = 22) autograft were tested approximately 9 months post‐ACLR. An uninjured control group (n = 22) was also tested on a single occasion. Phase‐specific bilateral absolute impulse asymmetries were calculated during the CMJ and compared between groups using Kruskal‐Wallis and post‐hoc testing. A linear regression model was used to assess the relationship between impulse asymmetries and isokinetic concentric knee extensor strength asymmetries. BPTB athletes demonstrated greater impulse asymmetries than HT athletes during the eccentric (P = 0.01) and concentric (P = 0.008) phases of the jump. Isokinetic strength asymmetry was a significant predictor of CMJ concentric impulse asymmetry in both BPTB (r2 = 0.39) and HT athletes (r2 = 0.18) but not eccentric impulse asymmetry in any group. The greater loading asymmetries demonstrated by BPTB than HT athletes 9 months after ACLR may contribute to the differing incidence rates of contralateral ACL injury. The findings suggest that graft‐specific loading asymmetries should be targeted during rehabilitation prior to RTP.

Влияние срока после травмы на результат восстановления передней крестообразной связки М. Л. Головаха, И. В. Диденко Запорожский государственный медицинский университет. Украина © Головаха М. Л,, Диденко И. В., 2019 Операція з відновлення ушкодженої передньої схрещеної зв'язки (ПСЗ) -одна з найчастіше виконуваних в ортопедії. Проте питома вага невдач залишається на рівні 5-15 %, а відсоток повного відновлення фізичної активності пацієнтів становить 60-85 %. Основних причин незадовільних результатів насправді дві: ушкодження трансплантата з формуванням хронічної нестабільності та розвиток посттравматичного артрозу. Мета: визначити вплив терміну відновлення ПСЗ на ризик розвитку посттравматичного гонартрозу. Методи: проаналізовано віддалені результати лікування 527 пацієнтів, прооперованих від 2 до 16 років тому, за шкалою IKDC -у 367 (69,64 %). Оцінено прогресування артрозу на підставі класифікації Келгрен-Лоуренс у 225 хворих (42,69 %). Варіаційно-статистичний аналіз виконано з використанням програми Statistica, версія 13 (TIBCO Software Inc., Ліцензія JPZ804I382130ARCN10J). Результати: у пацієнтів, прооперованих у перші 2 міс. після травми, результат лікування за шкалою IKDC виявилися найкращими (до групи А -повністю норма -віднесено 68,9 %). В осіб, яким реконструкцію ПСЗ проведено в термін від 2 міс. до 2 років після травми, показник за шкалою IKDC був на 8 % меншим (р < 0,01). А серед пацієнтів, прооперованих через 2 і більше років після травми, до групи А за шкалою IKDC віднесено лише 48,0 % випадків (р < 0,01). У групі пацієнтів, яким ПСЗ відновлено до 2 міс. із моменту травми, визначено мінімальне прогресування гонартрозу (20,4 %); у термін від 2 міс. до 1 року -39,1-40,6 %; понад року -57,1-68,8 %. Статистично доведено залежність прогресування гонартрозу від терміну, що пройшов з моменту травми до хірургічної реконструкції ПСЗ. Висновки: прогресування гонартрозу залежить від терміну, що минув від моменту травми до хірургічного втручання з відновлення ПСЗ. Пацієнти, прооперовані в перші 8 тижнів після травми, мають достовірно меншу ймовірність прогресування гонартрозу. Outcomes of Anterior Cruciate Ligament Reconstruction in Females Using Patellar–Tendon–Bone versus Hamstring Autografts: A Systematic Review and Meta-Analysis

The current review aims to compare the outcomes of anterior cruciate ligament (ACL) reconstruction in the female population after patellar-tendon-bone and hamstring grafts. The review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. All original randomized controlled trials and prospective cohort studies that compared clinical outcomes after female ACL reconstruction using hamstring versus patellar-tendon-bone grafts were included. All clinical outcomes reported by three or more studies were included. Fifteen publications, with 948 female patients, were included. Most outcomes were reported to have no significant graft differences by all studies that reported the outcome. These included all the outcomes for objective knee scores (International Knee Documentation Committee [IKDC] objective knee score), neuromuscular testing (quadriceps strength, hamstring strength, and single hop test), graft rupture or failure, and subjective knee scores (Lysholm score and IKDC subjective knee score). The pivot shift test, flexion deficit, and presence of crepitus were also reported to have no significant graft differences by all studies. Some studies reported a significant difference in anteroposterior laxity (Lachman's test and instrumented laxity), range of motion deficits (extension deficit), and sports and activity level (Tegner score). However, these statistically significant differences were noted to be clinically insignificant due to the normal population variation or standard error of measurement of these tools of evaluation. Patients reconstructed with patellar-tendon-bone grafts have a higher risk of kneeling pain. There was no significant difference in the incidence of crepitus. Most of the outcomes following female ACL reconstructions showed no clinically and statistically significant difference when either patellar-tendon-bone or hamstring autograft was used. These included outcomes for anteroposterior laxity, objective knee scores, neuromuscular testing, graft rupture or failure, subjective knee scores, sports and activity level, and crepitus. This a level II study.
