We report a case of Ewings sarcoma (ES) involving the patella in a young female. ES of patella is a rare entity. The patient was presented with anterior knee pain and swelling arising from the patella. She was treated with neoadjuvant chemotherapy followed by wide excision of the patella and reconstruction of the extensor mechanism using split tendoachilles auto graft. The patella is an uncommon site for primary or metastatic tumors of the bone. ES, though rare, should be included in the differential diagnosis of swellings arising from the patella. Auto graft from the tendoachilles is a good alternative for reconstructing the extensor mechanism of the knee. Core tip: We are presenting an article of Ewings sarcoma (ES) of the patella-a rare entity treated by a novel technique. The common reported tumors of the patella include chondroblastoma, giantcell tumor, osteosarcoma, metastasis, etc . We could not find any report on a single case of ES/PNET in the patella treated with patellectomy and extensor mechanism reconstruction using tendoachilles auto graft in the whole of English bio-medical literature. We present here a case of ES of the right patella in a 26 years old female. All reported extensor mechanism reconstructions after patellectomy were by tendoachilles allografts. ÖZBu makalede tipik olmayan bir bölgede meydana gelmesi ve yumuşak doku tutulumu olması nedeni ile radyolojik ve klinik olarak benzersiz olan bir patellar dev hücreli tümör olgusu sunuldu. Yirmi dört yaşında hasta kliniğimize diz önü ağrısı ile başvurdu. Radyolojik incelemeler ve perkütan iğne biyopsisi ile patellar dev hücreli tümör tanısı konuldu. Hastada total tutulan yumuşak doku komponentlerini de içerecek şekilde patellektomi ile lezyon en-blok olarak çıkarıldı ve iki adet Aşil tendon allogreft ile ekstansör mekanizma yeniden yapılandırıldı. Cerrahi sonrası tanı histopatolojik inceleme ile doğrulandı. On ikinci ayda hastada nüks yoktu ve diz eklem hareket açıklığı 110º fleksiyon, 30° ekstansiyon idi. Lysholm diz skoru 72 idi. 90º/sn. hızda izokinetik kas kuvvet testinde sağlam diz ile karşılaştırıldığında ekstansiyonda %51.4, fleksiyonda ise %21.1 kuvvet kaybı ölçüldü. Sonuç olarak, dev hücreli tümörler yıkıcı özellikleri ve yüksek nüks oranları ile tedavi edilmesi zor tümörlerdir. Kas gücü ve eklem hareket açıklığında azalma olmasına rağmen, çalışma sonuçlarımıza göre, cerrahi tedavi yöntemimizin patellanın dev hücreli tümörlerinin tedavisinde bir alternatif olabileceğini düşünmekteyiz.Anahtar sözcükler: Dev hücreli tümör; patella; rekonstrüksiyon. ABSTRACTIn this article, we report a case of a giant-cell tumor of the patella which occurred in an atypical location with soft tissue involvement, which makes our case clinically and radiologically unique. A 24-year-old patient was admitted to our clinic with a complaint of anterior knee pain. Radiological examination and percutaneous fine needle biopsy showed a diagnosis of giant-cell tumor of the patella. The patient underwent patellectomy with en-bloc resection of the lesion with totally involved soft tissue components and the extensor mechanism was reconstructed with two Achilles allografts. After surgery, the diagnosis was confirmed by histopathological examination. At 12 months, the patient had no recurrence with 110º flexion and 30° extension of the range of motion of the knee. Lysholm knee score was 72. At 90°/sec isokinetic muscle strength test, the muscle strength loss was found to be 51.1% in the extensor and 21.1% in the flexor, compared to non-involved knee. In conclusion, giant cell tumors are difficult to treat, as they are destructive lesions with high recurrence rates. Despite reduced range of motion and muscle strength, based on our study results, we believe that our surgical treatment modality may be an alternative for the treatment of patellar giant cell tumors.Keywords: Giant cell tumors; patella; reconstruction. Abstract: We report 15 cases of primary tumours and pseudotumours of the patella. The radiographic features and the surgical treatment of the different histological types are described in relation to Enneking's surgical staging and data derived from the literature.
