Background: Heart rate (HR) reduction is associated with improved outcomes in patients with heart failure (HF) and biomarkers can be a valuable diagnostic tool in HF management. The primary aim of our study was to evaluate the short-term (6 months) effect of ivabradine on N-terminal pro B-type natriuretic peptide (NT-proBNP), CA-125, and cystatin-C values in systolic HF outpatients, and secondary aim was to determine the relationship between baseline HR and the and , and cystatin-C were taken at baseline and at the end of a 6-month follow-up in both groups. Results: There was a significant decrease in NYHA class in the ivabradine group (2.67 ± ± 0.47 vs. 1.85 ± 0.61, p < 0.001). When ivabradine and control groups were compared, a significant difference was also found in NHYA class 6 months later (p = 0.013). A significant decrease was found in HR in the ivabradine and control groups (84.10 ± 8.76 vs. 68.36 ± ± 8.32 bpm, p = 0.001; 84.51 ± 10 vs. 80.40 ± 8.3 p < 0.001),353.02 ± 1,453.77 vs. 717.81 ± 834.76 pg/mL, p < 0.001) in the ivabradine group. Introduction: Serum levels of CA125 are often high in advanced heart failure (AHF) patients. Aim: To determine the predictive value of CA125 in forecasting the occurrence of death or cardiac transplantation in an AHF population. Methods: 88 AHF patients referred for heart transplantation were divided into 2 groups based on CA125 levels: normal (CA125 b 38 U/mL) and elevated (≥ 38 U/mL). Events (death or heart transplant) were monitored over a period of 13 ± 7 months after CA125 determination. Results: Patients with elevated CA125 (n = 65) had significantly lower blood pressure, body mass index, serum sodium and peak exercise oxygen consumption, while B-type natriuretic peptide levels were significantly higher. The combined primary endpoint (death or heart transplant) rate was 39.4% and 62.3% in normal and elevated CA125 groups, respectively (p = 0.029). Multivariate regression analysis showed that CA125 and sodium levels were the only independent predictors of the combined endpoint. Conclusion: In AHF patients, plasma CA125 was an effective prognostic marker. Its determination may contribute to better risk stratification in this population.

Purpose: The aim of this study was to analyze associations between serum cancer antigen 125 (CA 125) levels and left ventricular (LV) function in patients with end-stage renal disease on maintenance hemodialysis (HD). Methods: CA 125 levels, pro-brain natriuretic peptide (pro-BNP) and biochemical parameters were measured, and echocardiography was performed for 110 patients and 47 healthy controls. Results: The mean CA 125 level in patients, 38.78 AE 35.48 U/ mL, was significantly higher than that found in healthy controls (9.20 AE 4.55 U/mL; p ¼ 0.003). Patients with elevated CA 125 levels (n ¼ 40) had significantly lower levels of albumin and reduced relative wall thickness, LV ejection fraction (EF) and fractional shortening but significantly higher levels of pro-BNP and a greater left ventricular end-diastolic diameter (LVEDd) and -systolic diameter (LVESd). CA 125 levels were positively correlated with pro-BNP (r ¼ 0.596, p50.05) and C-reactive protein (CRP) levels (r ¼ 0.439, p50.05), as well as LVEDd (r ¼ 0.599, p50.001), LVESd (r ¼ 0.750, p50.001) and LV mass index (r ¼ 0.378, p50.05). In contrast, serum CA 125 levels were negatively correlated with albumin (r ¼ À0.513, p50.05) and hemoglobin (r ¼ À0.475, p50.05) as well as the EF (r ¼ À0.878, p50.0001). A depressed EF ( ¼ À1.121, p50.0001) and increased CRP levels ( ¼ 0.247, p ¼ 0.035) were independent predictors of high CA 125 levels in the whole group in the multivariate-model. Conclusions: Our study is the first to demonstrate an association between serum CA 125 levels and LV systolic dysfunction via inflammation in patients on maintenance HD.
