ÖzetTümör ilişkili antijenler, malignitelerde takip ve tedaviye yanitin değerlendirilmesinde kullanilir. Antijenler inflamatuar hücreler tarafindan eksprese edilebilir ve malin hücrelerin adezyon ve metastazlarinda önemli rol oynabilir. İnflamatuar romatizmal hastaliklarin akciğer tutulumu ve plevral effüzyonu bulunan hastalarda tümör ilişkili antijen seviyelerinin arttiği rapor edilmiştir. Biz, RA tanili bir kadin hastada tümör belirteçleri ile hastalik aktivitesi arasindaki ilişkiyi sunmayi amaçladik. Bir yildan beri RA tanisi olan 61 yaşinda kadin hasta, sağ dizde, her iki el bileğinde ve metakarpofalangeal eklemlerde şişlik ve hassasiyet olmasi nedeniyle kliniğimize başvurdu. Özgeçmişinde leflunomid 20 mg/gün ve sulfasalazin 2 gr/gün kullanimi dişinda bir özellik yoktu. Fizik muayenede bilateral akciğer alt zonda raller mevcuttu. Hastanin laboratuvar verileri: hastalik aktivite skoru : 6,02, eritrosit sedimentasyon hizi [ESH]: 72 mm/h, CRP: 37,95 mg/l, romatoid faktör: 48,5 IU/ml, anti-CCP: 67,1 U/ml, CA19-9: 410,67 U/ml [0-37 U/ml], CA-125: 197,1 U/ml [0-35 U/ml] ve CA-15-3: 51,6 U/ml [0-31,3 U/ml] idi. Postero-anterior akciğer grafisinde bilateral orta ve alt zonlarda retiküler dansite artişi ve toraks BT'de her iki akciğerde retiküler, buzlu cam opasiteleri ve ince bal peteği değişiklikleri görüldü. Malignite lehine herhangi bir kanit yoktu ve aktif hastaliği olan hastaya anti-inflamatuar tedavi verildi. Takiplerde, ESH, CRP, DAS-28, CA19-9, CA-125 ve CA15-3 düzeyleri geriledi. RA' da hastalik aktivitesini, organ hasarini ve tedaviye yaniti belirlemek için ideal biyobelirteçlere ihtiyaç vardir. Bağ doku hastaliklarinda organ tutulumu ile tümör belirteçleri arasindaki ilişki nedeniyle, bu biyobelirteçler hastalik progresyonu ve tedaviye yanitin değerlendirilmesinde yardimci olabilir. Tümör belirteçleri, malignite ekarte edildikten sonra, RA hastalarinda (özellikle akciğer tutulumlu) hastalik aktivitesinin değerlendirilmesinde kullanilabilir. Structural and functional characteristics of mucins and cytokeratins are shortly described. Thereafter, those commonly used in breast cancer as serum tumor markers are considered. First CA15.3, MCA, CA549, CA27.29 mucins and CYFRA21.1, TPA, TPS cytokeratins alone or in association have been examined in different stages and conditions. Then their usefulness in monitoring disease-free breast cancer patients is evaluated. The central role of the established cut-off and critical change, the "early" treatment of recurrent disease and the potential benefit in survival are other issues that have been highlighted and discussed. The successive sections and subsections deal with the monitoring of advanced disease. In them, the current recommendations and the principal findings on using the above mentioned mucins and cytokeratins have been reported. A computer program for interpreting consecutive measurements of serum tumor markers also has been illustrated. The final part of the chapter is devoted to mucins and cytokeratins as markers of circulating and disseminated tumor cells and their usefulness for prognosis.
