Background: Prostate tuberculosis (PTB) is one of extra pulmonary tuberculosis which potentially has more frequent fatal complications and more severe quality of life deterioration. It is a very rare disease, with prevalence of 2.6% of all urogenital tuberculosis (UGTB). Prostate tuberculosis may be a sexually transmitted disease and leads to sexual dysfunction. Case: Male, 54 years old, with urinary retention, dysuria, flank pain in the last 1-month, recurrent urinary tract infection in the past 1 year and decrease body weight of 8 kg in 1 month. Physical examination demonstrates enlarged prostate. Patient was referred to Wangaya Hospital with benign prostate hyperplasia (BPH) and suspect malignancy. After underwent clinical and supporting examination, patient underwent TURP procedure. Histopathology examination revealed PTB. Patient was then treated with first line anti-tuberculosis drug (ATD). Discussion: Multiple risk factors are involved in TB disease. PTB spread occur through hematogenous, lymphatic, or direct routes. Clinical features and supporting examinations of PTB are non-specific. Diagnosis often made through incidental histology finding post TURP. Standard ATD regiment administered based on World Health Organization (WHO) guideline. Duration can be prolonged due to suboptimal concentration to prostate tissue. Conclusion: Multidisciplinary approach for extra pulmonary TB is needed. Thorough history taking, and high index of suspicion are important aspects. PTB diagnosis should be considered in patients with recurrent lower urinary tract symptoms refractory to standard therapy in TB endemic area.

Prostate tuberculosis (PT) is not uncommon, but rarely diagnosed. Treatment of PT is a difficult task, since it is difficult to achieve an adequate concentration of antibacterial drugs in the parenchyma of even a healthy organ, and in the case of the formation of prostate caverns, their fibrous walls almost completely prevent the penetration of anti-tuberculosis drugs into the destruction focus. The method of combined surgical treatment of PT is described in detail on the example of the patient. During polychemotherapy, the cavity is opened by means of transurethral electroresection, followed by coagulation of the cavity wall by radiation of a high-energy diode laser with a wavelength of 940 nm and a power of 150 W. This approach allows to cleanse the prostate cavity from purulent-necrotic detritus and interrupt the pathological infectious and inflammatory process in its wall due to coagulation with laser radiation. The proposed method of surgical treatment of PT is both radical, since the walls of the cavities are excised and coagulated, and is minimally invasive, therefore it can be recommended for wider application.

Objectives:Tuberculosis (TB) of the prostate is a very rare disease. Most urologists are not familiar with it. Here, we present our experience with five cases of this disease and a review of literature.Materials and Methods:This is a retrospective study in a tertiary care center (IGIMS, Patna, Bihar, India) from January 2013 to February 2018.Results:All the patients were in their fourth to sixth decades of life. Lower urinary tract symptom was the most common presentation, predominantly irritative symptoms. A history of pulmonary TB was present in one case. Four out of five cases (80%) had a suspicious prostate on the digital rectal examination (DRE) (hard in consistency). Serum prostate-specific antigen (PSA) level was slightly elevated with a mean of 13.24 ng/ml. Urine analysis revealed sterile pyuria in all patients, and the urine culture was negative. The urine for acid-fast bacilli was positive in one case (20%). Mycobacterium tuberculosis culture test was positive in two cases (40%). The transrectal ultrasonogram showed hypoechoic areas with irregular outlines in three cases (60%) and calcification in two cases (40%). Histopathological examination showed chronic granulomatous prostatitis with few Langhans-type giant cells in four cases (80%). All patients were scheduled to receive 6 months of chemotherapy with isoniazid, rifampicin, and ethambutol or pyrazinamide.Conclusion:TB of the prostate may present like cancer prostate with raised serum PSA and suspicious prostate on DRE; hence, a high index of suspicion with a wide range of investigations may be required to achieve a complete diagnosis of prostatic TB.
