Tuberculoid lymphadenitis due toMycobacterium chelonei

Abstract: An 8-year-old boy developed cervical lymphadenitis four months after an injection of dental anaesthetic. Histology of the lymphatic tissue showed a tuberculoid granulomatous reaction with scanty acid-fast bacilli.Mycobacterium cheloneiwas isolated in pure culture (NCTC 10882). The patient showed specific skin hypersensitivity to an extract ofM. chelonei, but not to that ofM. ranae. This is thought to be the first recorded case of lymphadenitis in man caused byM. chelonei; it adds another possibility to be consi… Show more

“…Our case is peculiar in that in addition to abscesses, typical tuberculoid granulomas with or without central necrosis constituted a dominant feature of the lesion. Tuberculoid granulomas had been observed in the few reports of deep-seated and disseminated forms of M. chelonei infections such as cervical lymphadenitis (Mooris et al, 1973), thyroiditis (Gutman et al^ 1974) and osteomyelitis (GraybiH et al, 1974;Jauregui, Arbulin & Wilson, 1977). What determines the types of tissue response to M. chelonei infection is not clear.…”

“…The resistance of M. chdonei to most standard anti-tuberculous drugs is well recognized (Mooris et al, 1973;Tsukamura et al, 1973;Altman et al^ 1975;Foz et al, 1978). Although Inman et al (1969) suggested that spontaneous resolution of cutaneous abscesses due to M. chelonei may occur, sporadic fatal cases of disseminated mycobacteriosis resistant to virtually all anti-tuberculous and antimicrobial treatments have been described (Graybill et a!., 1974;Jauregui et ai, 1977).…”

“…The clinical significance of Mycobacterium chelonei as a potential pathogen has not been clearly defined. However, this ubiquitous acidfast bacillus has been isolated from patients with endocarditis (1,10,14), thyroiditis (7), lymphadenitis (11), osteomyelitis (8), injection abscesses (4), cutaneous lesions involving the cervico-facial region (2), and lung lesions (5,6,19). The organism has also been isolated from porcine prosthetic heart valves (10,20), a monkey (9), a manatee (3), and a fish aquarium (13).…”
