Tube cecostomy and staged resection for obstructing carcinoma of the left colon

Abstract: Over a 17-year period, all patients presenting with acute obstruction of the left colon due to carcinoma were treated by emergency tube cecostomy. There were 57 patients aged between 35 and 93 years. After the decompressive procedure, eight died, 34 had complications, and eight were left with permanent cecostomies. Forty-one underwent secondary procedures of which 35 had resections. Seven patients died postoperatively and 15 had complications. Of the 34 survivors, in 23 the cecostomy closed spontaneously, and … Show more

“…In patients with complete bowel obstruction caused by cancer of the left colon, particularly tumors of the descending colon or sigmoid colon, transanal drainage is difficult, and emergency surgical decompression is often required (14)(15)(16). When curative resection of the tumor, including lymph node dissection, is performed at the initial operation, it is almost always necessary to fashion a temporary loop ileostomy or right transverse colostomy in addition to performing resection of the primary tumor and anastomosis (17)(18)(19).…”

“…Moreover, when emergency surgery is performed without preoperative bowel lavage, since the bowel contents are not fecal, it is relatively rare for surgeons to feel hesitation about performing anastomosis after one-stage resection of the tumor. Therefore, a complete obstruction of the left colon by cancer is a more serious problem (15)(16)(17)(18). At initial surgery, the radical resection of the primary tumor usually involves the removal of the cancer, followed by bowel anastomosis plus a temporary loop ileostomy and/or right transverse loop colostomy (19)(20)(21).…”

“…The surgical procedures employed can be classified into those involving resection of the primary tumor and procedures that only relieve obstruction without removing the primary lesion (16)(17)(18) is followed by anastomosis/reconstruction with construction of a covering colostomy in the right transverse colon, investigation of other tumors and diseases cannot be performed. In addition, the long operating time and highly invasive surgery make it a risky emergency operation (19)(20)(21).…”
