Trichoid Basal Cell Carcinoma Found in a Dilated Pore on the Nose

Abstract: A pore of Winer is a common, benign epidermal appendageal tumor arising from a hair follicle. We present a case where routine removal of a pore of Winer revealed the presence of a trichoid basal cell carcinoma. We describe the history of these lesions, discuss clinical and histologic differential diagnoses, and propose an explanation for the presence of these tumor cells. No prior report of basal cell carcinoma within a pore of Winer has been documented in the literature.

“…A few basaloid aggregations, including all of the squamous eddy‐like aggregations adjacent to the cornifying areas, were also positive for CKs 1 and 10, suggesting a close relationship between the infundibular component and basaloid aggregations in this BCC. This case is somewhat similar to the architecture recently reported for cases of BCCs with a central large pore (27, 28), although the clinical feature of this case was a nodular lesion with a central, keratotic plug. Interestingly, in two cases (Cases 2, 3), the inner parts of most of the basaloid aggregations that were without cornifying areas were also positive for CKs 1 and 10, suggesting differentiation toward the infundibulum as a whole lesion in these cases.…”

“…1,2 No previous reports of trichoblastoma, 3 which is a benign counterpart of BCC, with a dilated pore have been found. We herein report a case of trichoblastoma with a large central pore plugged with keratinaceous debris.…”

“…5 - 7 Each patient had one of the following concomitant diseases: squamous cell carcinoma, trichoblastoma, and trichoid basal cell carcinoma. 8 - 10 Pathologically, a dilated infundibulum filled with keratinous material opens into the surface and extends into the superficial or deep dermis. The lining epithelium is atrophic superficially but proliferative deep in the cavity.…”
