We herein report two cases of cystic trichoblastoma with an immunohistochemical study. The histopathological findings in these cases included new information, namely, their being composed of two and three cysts, the cystic components had features of a steatocystoma and a hybrid cyst in one case, and there were projections of an aggregation of trichoblastoma, as well as papillary projections of follicular germinative cells, from the cyst walls. The follicular germinative cells observed in the papillary projections and in the aggregations of trichoblastoma expressed cytokeratin-15 (clone C8/144B) and PHLDA1, markers of follicular stem cells. Cystic trichoblastoma is a unique type of trichoblastoma, which originates from the cyst walls of an infundibular cyst (usually) or steatocystoma/hybrid cyst (rarely). In some cases, a trichoblastic infundibular cyst is considered to be a minor form (or possibly a primitive stage) of cystic trichoblastoma.

“…Otras manifestaciones clínicas inusuales comunicadas en las revistas médicas incluyen nódulos con un poro central dilatado ocupado por queratina 9 ; formas gigantes mal delimitadas en localizaciones poco frecuentes como glúteos 10 o escroto 11 , o como una lesión pigmentada (violácea) en el muslo que simula un angioqueratoma o un melanoma nodular 1 . Al revisar la base de datos se identificó un informe de caso de tricoblastomas múltiples, que correspondía a una niña de siete años con múltiples pápulas claras, menores de 4 mm, localizadas en el dorso nasal; estas lesiones habían crecido con lentitud en los tres años previos, si bien en clínica se consideró el diagnóstico de tricoepiteliomas múltiples; la paciente no tenía antecedente familiar de lesiones similares y el estudio inmunohistoquímico realizado demostró positividad para citoqueratina AE1/AE3 y negatividad para citoqueratina 7, citoqueratina 20, CAM 5.2, antígeno carcinoembrionario, antígeno epitelial de membrana, receptor de andrógenos, S-100 y CD34 12 .…”

El tricoblastoma es un tumor anexial benigno poco frecuente que puede presentarse de novo o sobre lesiones preexistentes; su aparición más común se observa en el nevo sebáceo de Jadassohn. Su localización principal es la cabeza y en clínica se presenta como una pápula asintomática. También se han descrito casos en las publicaciones médicas de lesiones con formas gigantes, nódulos ulcerados y formas múltiples centrofaciales pigmentadas. El diagnóstico clínico e histopatológico es difícil de establecer debido a la similitud con otros tumores de anexos, en particular el carcinoma basocelular. El tratamiento de este tumor, cuando presenta una lesión única, es casi siempre la resección, pero en lesiones múltiples su atención es compleja.

Dilated pore was originally designated as a secondary or acquired trichoepithelioma by Winer, represents an appendageal tumor with differentiation towards hair structures. It is clinically characterized by having the appearance of a large blackhead filled with keratinous material. To the best of our knowledge, there has been no report of a dilated pore with four openings. We herein describe such a case.
