Trichilemmal carcinoma: an unusual presentation of a rare cutaneous lesion

Abstract: SUMMARYTrichilemmal carcinoma (TC) is a rare cutaneous neoplasm which is derived from adnexal keratinocytes, is histologically invasive, contains cytologically atypical clear cell neoplasm and is in continuity with the epidermis and/or follicular epithelium. However, the diagnostic criteria and even the existence of TC have been contentious. We report the case of a 92-year-old woman with TC of the head and neck region who presented with an unusually long history. She was treated successfully with wide local ex… Show more

“…7 Wide local excision has been suggested for TC but clear margins were mostly not described. 9 , 11 - 12 Mohs micrographic surgery has been reported to be a successful technique for treating malign trichilemmal tumor. 15 We treated all our patients with wide local excision with 1 cm.…”

“…Local cervical lymph node and distant metastases of TC have been rarely reported in the literature. 9 - 11 There is no optimal treatment for this neoplasm with metastases and there is no established chemotherapy regimen in the literature. We scanned our patients with cranial, cervical and thoracoabdominal CT except one because he had refused further investigations.…”

“…There is no current consensus on surgical margins or follow-up for patients with TC. Wilkie et al 2 suggested using margins for cutaneous squamous cell carcinoma, specifically those that are 4–6 mm in size. Zhuang et al 16 showed improved overall survival in their study when a surgical margin of 1 cm was taken.…”

“…The distinction between trichilemmal carcinoma and SCC with clear cell features has a practical significance since the risk of metastasis for trichilemmal carcinoma is very low, unlike SCC which has higher rates of local recurrence and metastatic potential 44 45. However, histologically, the differential diagnosis may be often challenging.…”
