Trends in mortality from coronary heart and cerebrovascular diseases in the Americas: 1970-2000

Abstract: Recent falls in CHD and CVA were less favourable in Latin America than in the USA and Canada. This may reflect unfavourable changes in nutrition (including obesity), physical activity, and smoking in most Latin American countries, together with less effective control of hypertension and management of the diseases.

“…The reasons for this trend are not totally known, but it can be attributed to better knowledge and management of risk factors. Even in middle and low-income countries there have been reports of cases of stroke reduction during this period, although less pronounced as compared to developed countries 1,10 . It is possible that, in Latin America, the reduction is related to better control of risk factors, but also to technological advances and improvement in the quality of health services 1,5 .…”

“…The group from 80 years had no statistically significant risk, but there were a small number of individuals. It is well established that the greater the age, the greater the risk for ischemic stroke 10 . However, in this population, the risk is already evidenced in an early age.…”

“…However, the decrease in stroke mortality is not equally distributed across the country and it occurs mainly on the poorest areas of the country and among Blacks 5,6,7 . Compared to previous mortality studies 8 , there are few studies evaluating the prevalence of stroke in Brazil according to sociodemographic characteristics 9,10,11,12 . These studies were carried out in cities or isolated regions reflecting more local realities than a national panorama of the disease.…”

“…Apesar da significativa redução da mortalidade hospitalar do IAM nas últimas décadas 8,9 , houve pouco avanço no conhecimento sobre a epidemiologia e o tratamento na fase pré-hospitalar. É notória a necessidade de mudanças nesse cenário, embora poucas sejam as evidências geradas para este fim.…”
