Az elmúlt néhány évtizedben a gyermeket vállaló nők életkora emelkedő trendet mutat. Számos tanulmány mutatott rá az anyai életkor emelkedésének és a terhesség kimenetelének összefüggésére a vetélés, a halvaszületés, a praeeclampsia, a terhességi hipertenzió, a gestatiós diabetes mellitus, a koraszülés, a kis vagy nagy gesztációs korú újszülött, valamint az elektív vagy sürgősségi császármetszés kapcsán. E vizsgálatok egymásnak ellentmondó megállapításokról számoltak be. A jelenlegi közlemény célja az, hogy összefoglalja az előrehaladott anyai életkor és a terhesség kimenetele közti, bizonyítékokon alapuló információkat. Orv. Several studies have assessed the association between advanced maternal age and adverse pregnancy outcome, including miscarriage, stillbirth, preeclampsia, gestational hypertension, gestational diabetes mellitus, preterm birth, delivery of a small-or large-forgestational-age neonates and elective or emergency Cesarean section. Számos tanulmány eredményei arra engednek következtetni, hogy az előre-haladott anyai életkor és a terhesség kimenetele -beleértve a vetélés, halvaszületés, praeeclampsia, terhességi hipertenzió, terhességi cukorbetegség (GDM), koraszü-lés, kis súlyú (SGA), valamint a nagy súlyú (LGA) újszü-lött és elektív vagy sürgősségi császármetszés -között szoros korreláció van. Egyes tanulmányok eredményei azonban ellentmondásosak [6,7,8,9,10,11,12,13,14,15]. A jelen összefoglalónk célja, hogy áttekintsük az anyai életkor és az anyai, valamint magzati komplikációk összefüggéseit.

Objective To examine the associations between extremes of maternal age (≤17 years or ≥40 years) and delivery outcomes.Design Retrospective cohort study.Setting Urban maternity hospital in Ireland.Population A total of 36 916 nulliparous women with singleton pregnancies who delivered between 2000 and 2011.Methods The study population was subdivided into five maternal age groups based on age at first booking visit: ≤17 years, 18-19 years, 20-34 years, 35-39 years and women aged ≥40 years. Logistic regression analyses were performed to examine the associations between extremes of maternal age and delivery outcomes, adjusting for potential confounding factors.Main outcome measures Preterm birth, admission to the neonatal unit, congenital anomaly, caesarean section.Results Compared with maternal age 20-34 years, age ≤17 years was a risk factor for preterm birth (adjusted odds ratio [adjOR] 1.83, 95% confidence interval [95% CI] 1.33-2.52). Babies born to mothers ≥40 years were more likely to require admission to the neonatal unit (adjOR 1.35, 95% CI 1.06-1.72) and to have a congenital anomaly (adjOR 1.71, 95% CI 1.07-2.76). The overall caesarean section rate in nulliparous women was 23.9% with marked differences at the extremes of maternal age; 10.7% at age ≤17 years (adjOR 0.46, 95% CI 0.34-0.62) and 54.4% at age ≥40 years (adjOR 3.24, 95% CI 2.67-3.94).Conclusions Extremes of maternal age need to be recognised as risk factors for adverse delivery outcomes. Severe ovarian hyperstimulation syndrome (OHSS) is a potentially life-threatening complication during assisted reproduction technology (ART). The aetiology of this condition is still not fully understood. Several gene variations in the FSH receptor (FSHR) gene have been identified for the very rare cases of spontaneous OHSS. There are only few published data on gene variations in sterility and iatrogenic OHSS and no data regarding aromatase (cytochrome P450 19A1; CYP19A1). Ninety-one ART patients with OHSS, eighty-eight ART patients without OHSS and ninety-seven women with assumed normal fecundity were analysed for the FSHR single nucleotide polymorphism (SNP) gene variations Asn680Ser (rs6166), Ala189Val, Ile160Thr, Thr449Ile (rs28928870) and the CYP19A1 rs10046 locus using real-time PCR.
