Treatment preferences in stage IA and IB testicular seminoma: multicenter study of Anatolian Society of Medical Oncology

Abstract: Our results indicate that adjuvant treatment with carboplatin or RT is associated with improved DFS compared with surveillance for men with stage I testicular seminoma after orchiectomy. Moreover, the treatment strategy is an important prognostic indicator for DFS and a predictive factor for relapse. Although adjuvant treatment, especially carboplatin, seems to be a suitable treatment for patients with risk factors for relapse, surveillance is still feasible and the preferred management option after radical or… Show more

“…About 33% are referred to surveillance regimens and only 5% receive radiotherapy [31]. Noteworthy, radiotherapy is still the most popular treatment for seminoma patients in the United States and in Eastern European countries [28], [32]. However, surveillance and carboplatin are slowly gaining shares [33].…”

“…3,10,11 Although the significance of these risk factors has been questioned in recent prospective studies, those studies lacked important clinical details in an appreciable fraction of cases. 17 Consequently, current practice has varied, 18,19,20,21 and a recent multicenter study has shown that approximately one quarter of patients with stage I seminoma received adjuvant chemotherapy. 13 In NSGCTs, LVI and/or tumor comprising a > 50% embryonal component are recognized as risk factors for relapse and distant metastasis.…”

“…In the group of 282 patients with seminoma in CS I, treated between 1997 and 2013, reported by Bilici et al ., 130 (46.1%) patients received PORT. During the five-year follow-up, the OS of the AS group was lower when compared with adjuvant treatment patients, but statistical difference was not demonstrated [ 18 ].…”
