Radiotherapy in testicular germ cell tumours – a literature review

Abstract: Testicular germ cell tumours (GCT) represent about 1–2% of malignant in men. The essential therapeutic option for early-stage GCT is radical orchiectomy (RO), except in situations that require immediate chemotherapy in patients with a massive dissemination and unequivocally elevated levels of tumour markers.Postoperative radiotherapy (PORT) in patients with testicular seminoma in Clinical Stage I (CS I) is one of the treatment options next to active surveillance (AS) and chemotherapy (CHTH). Regardless of the … Show more

“…Taking CS into consideration, PORT showed no advantage in reducing the risk of CSM in each CS. Based on previous studies and improvement of multi-modal management regimens, current utilization of radiotherapy may have come to a crossroad, adjustment has become a challenge for urologists and oncologists [7].…”

“…Most patients with TS can be cured with orchiectomy and, if necessary, subsequent chemotherapy or Post-Operative Radiotherapy (PORT) [1]. Although accurate diagnosis and advanced multimodal treatment significantly contribute to good prognosis, controversies regarding various side effects of PORT for the treatment of TS and its efficacy on patients' long term survival still exists [1,[3][4][5][6][7].…”

“…26 Radiation remains one of the preferred treatment approaches for early stage seminoma even in the modern era. 27 Traditionally, radiation portals encompassed the para-aortic, ipsilateral iliac and obturator nodes in a 'dog-leg' (DL) field to a dose of 30Gy. Although local control rates were excellent, long term data provided concern regarding late sequelae.…”

“…There have been many advances in technologies underpinning the delivery of radiotherapy, including intensity-modulated radiotherapy (IMRT), volumetric-modulated arc therapy (VMAT), and proton therapy (PT). There are case reports that suggest, for example, that IMRT can lead to a reduction in acute affects [41], and there is initial evidence that VMAT also reduces acute affects with PT reducing the risk of secondary malignancies in younger patients [42]. However, robust and largescale clinical trial evidence is lacking in this area, and hence accurate survey and meta-analysis studies are not yet possible.…”
