Treatment outcome and patterns of failure in patients of pinealoblastoma: review of literature and clinical experience from a regional cancer centre in north India

Abstract: Maximal safe resection followed by craniospinal irradiation and systemic chemotherapy with 6 cycles of carboplatin-etoposide regimen is a reasonable treatment strategy in patients of pinealoblastoma more than 8 years of age in a developing nation. However, the same strategy is less effective in younger children and innovative study designs of intensification of post-operative treatment must be explored in this age group.

“…[63] For 17 patients with pinealoblastoma, the 2 years actuarial overall survival have been reported to be 85.6%. [64] Leptomeningeal spread at diagnosis was seen in 4 patients during presentation and age more than 8 years, and no metastasis were significant predictors for better relapse-free survival. For 44 patients with primary CNS lymphoma treated at TMH, overall survival was found to be 83% (ASNO 2013) treated with high-dose methotrexate and RT.…”

“…Lee et al did not find any impact of CT on survival, but the analysis was limited by the small number of patients and the many different regimens used [10]. In a series of 17 PBs by Biswas et al, the most common regimen was a combination of Carboplatin and Etoposide [17]. Brandes et al suggest that patients should be stratified according to recurrence risk when planning adjuvant treatment [18].…”

“…3 – 5 As a result of such an exceedingly low incidence of PBs in the general population, considerable dispute over factors affecting the prognosis and treatment strategies was present. 6 , 7 In addition, the relationship of clinical behaviors between age-related subgroups was not identified. Therefore, this study aimed to explore adverse factors of overall survival (OS), multimodal treatment outcomes, and different clinical distributions between pediatric and adult patients based on our experience with surgical treatment in an institution.…”
