Treatment of patent ductus arteriosus: indomethacin or ibuprofen?

Abstract: Persistent patent ductus arteriosus (PDA) in preterm infants can result in serious hemodynamic changes causing respiratory, gastrointestinal and renal morbidities if not treated within the first week of life. The treatment options available are a conservative approach, pharmacological treatment with cyclo-oxygenase (COX) inhibitors and surgical ligation. The COX inhibitors approved for use in the United States are indomethacin and ibuprofen lysine. Both of these drugs are equally effective in closing the PDA. … Show more

“…17 Indomethacin reduces the blood flow velocity in kidneys, intestine, and brain, ibuprofen has not this effect. 1,14 In infants who received 2 or more courses, the decrease in urine output and the increase in serum creatinine concentration were not different between ibuprofen and indomethacin.…”

“…Chan et al 10 compared of therapy: an early onset of treatment increases the chances of success. 1 At the time of writing, ibuprofen is becoming the first choice for ductal closure due to its higher association with cerebral, gastrointestinal and renal blood flow velocity, whereas ibuprofen does not exhibit this effect. 2 Renal failure is more common in infants receiving indomethacin compared to ibuprofen.…”

“…[3] The success of treatment with intravenous medication has been questionable in some cases. [3] Thus, over the last three decades, several techniques of catheterization closure have been described. [4] The success rate is significantly higher for PDA if it has a diameter smaller than 2.5 mm.…”

“…5 Penutupan DA secara spontan hanya terjadi pada 34% BKB. 6,7 Penutupan spontan DAP tanpa sindrom gawat nafas pada BKB sebesar 10% pada hari ke-1, 60% hari ke-2, 85 % hari ke-3 dan 97% hari ke-4. Apabila penutupan spontan tidak terjadi pada hari ke-4 maka akan menjadi duktus arteriosus persisten.…”
