Treatment of malignant fibrous histiocytoma of the heart

Abstract: Primary soft tissue sarcoma of the heart is encountered infrequently in clinical practice. Treatment is reported of an intracardiac malignant fibrous histiocytoma, consisting of maximal surgical resection followed by 5600 cGy of conventionally fractionated radiation therapy. Transesophageal echocardiograms done during follow‐up were useful in assessing tumor control. Aspects of patient care are discussed in conjunction with a review of the available literature. Cancer 1992; 69:956–961.

“…Surgery is the only treatment capable of improving outcome even after palliative resection, but long-term survivors are still anecdotal. Sporadic cases of long-term survivors treated with radiation therapy (Stevens et al 1992) and/or chemotherapy (Potter et (Demny. 1996).…”

“…Recommended therapy has often been complete or partial excision of the cardiac malignancy, followed by adjuvant radiation therapy and/or chemotherapy [3,32]. The literature contains reports of responses of inoperable cardiac and great vessel sarcoma to radiation therapy 1391, chemotherapy [40], and combination therapy [41], but experience is limited by the rarity of these malignancies. Doxorubicin chemotherapy and radiation therapy are cardiotoxic.…”

“…In a case report of a patient with cardiac MFH, a 5,600 cGy dose of radiotherapy was delivered in conjuction with resection and provided clinical remission for 16 months. 13 The role of heart transplantation in the treatment of primary cardiac sarcomas remains controversial. Our patient is the second identified in the literature to undergo orthotopic heart transplantation for primary cardiac MFH.…”
