Treatment of Macular Seborrheic Keratoses Using a Novel 1927-nm Fractional Thulium Fiber Laser

Abstract: Nonablative fractional resurfacing using the 1927-nm thulium fiber laser is a safe and effective treatment modality to decrease nonfacial macular SK.

“…More pronounced issues with acne scarring, skin texture, dyschromia as well as pore size can be simultaneously improved with low‐density “mini” treatments utilizing 1,550‐nm erbium glass (Fraxel Re:store, Solta Medical) over six sessions as discussed previously (see Atrophic Acne Scarring ) . When patients present predominantly with discrete or diffuse, mottled superficial dyspigmentation characterized mainly by macular seborrheic keratoses and solar lentigines, the fractionated 1,927‐nm thulium fiber laser (Fraxel Dual, Solta Medical) has proven to be a treatment of choice . The 1,927‐nm wavelength can be combined with 1,550‐nm to achieve both superficial and deep treatment depths, respectively, in a single session.…”

“…1 Evaluation of six recruited patients who underwent 3 monthly laser sessions revealed >50% reduction in overall global SKs 1 month after treatment, with even more-pronounced improvement at 6-month follow-up. In their pilot clinical study, Polder and colleagues investigated the use of a 1927-nm thulium fiber laser in the clearance of macular SKs.…”

“…62 In addition, there is a report of successful treatment of macular seborrheic keratoses with the 1927-nm NAFL. 64 While the improvement in pigmentation, fine rhytides, and overall texture is well established with NAFLs, there has been some controversy over whether the NAFLs truly have a skin-tightening effect, despite several studies reporting this benefit. 1,65,66 A true skin-tightening effect comes from dermal fibroplasia and wound contraction, thought to be at least partially induced by heat shock protein 70.…”
