Background: Dermal scaffolds have created a paradigm shift for burn and wound management by providing improved healing and less scarring, while improving cosmesis and functionality. Dermal regeneration template (DRT) is a bilayer membrane for dermal regeneration developed by Yannas and Burke in the 1980s. The aim of this review is to summarize clinical evidence for dermal scaffolds focusing on DRT for the management and reconstruction of burn injuries and complex wounds. Methods: A comprehensive search of PubMed was performed from the start of indexing through November 2022. Articles reporting on DRT use in patients with burns, limb salvage, and wound reconstruction were included with focus on high-level clinical evidence. Results: DRT has become an established alternative option for the treatment of full-thickness and deep partial-thickness burns, with improved outcomes in areas where cosmesis and functionality are important. In the management of diabetic foot ulcers, use of DRT is associated with high rates of complete wound healing with a low risk of adverse outcomes. DRT has been successfully used in traumatic and surgical wounds, showing particular benefit in deep wounds and in the reconstruction of numerous anatomical sites. Conclusions: Considerable clinical experience has accrued with the use of DRT beyond its original application for thermal injury. A growing body of evidence from clinical studies reports the successful use of DRT to improve clinical outcomes and quality of life across clinical indications at a number of anatomical sites.

Negative pressure wound therapy is a widely used treatment for chronic, nonhealing wounds. Surprisingly, few studies have systematically evaluated the cellular and molecular effects of negative pressure treatment on human skin. In addition, no study to date has directly compared recently available single‐use negative pressure modalities to traditional negative pressure devices in a controlled setting. Here we developed a novel large‐scale ex vivo human skin culture system to effectively evaluate the efficacy of two different negative pressure wound therapy modalities. Single‐use and traditional negative pressure devices were applied to human ex vivo wounded skin sheets cultured over a period of 48 hours. Cellular tissue response to therapy was evaluated via a combination of histological analysis and transcriptional profiling, in samples collected from the wound edge, skin adjacent to the wound, and an extended skin region. Single‐use negative pressure wound therapy caused less damage to wound edge tissue than traditional application, demonstrated by improved skin barrier, reduced dermal‐epidermal junction disruption and a dampened damage response. Transcriptional profiling confirmed significantly less activation of multiple pro‐inflammatory markers in wound edge skin treated with single‐use vs traditional negative pressure therapy. These findings may help to explain the greater efficacy of sNPWT in the clinic, while offering a noninvasive system to develop improved NPWT‐based therapies.

As queimaduras são definidas como lesões dos tecidos orgânicos, ocasionados por agentes químicos, físicos e biológicos que comprometem as estruturas corporais causando morte celular, sendo no Brasil, uma das principais causas de mortalidade e incapacidade parcial ou definitiva. Objetivo: Destacar os principais cuidados de enfermagem às vítimas de queimaduras. Metodologia: Trata-se de uma revisão da literatura com abordagem qualitativa, onde o levantamento de dados para a pesquisa foi realizado no mês de janeiro de 2024, mediante as bases de dados MEDLINE, LILACS e BDENF, realizado pelo portal Biblioteca Virtual em Saúde (BVS). Utilizando-se dos Descritores em Ciências da Saúde (DeCS): "Cuidados de enfermagem", "Queimaduras" e ''Unidade de queimados". Foram incluídos artigos originais referentes ao tema assistência ao paciente adulto queimado, disponíveis eletronicamente e gratuitos, no período de 2020 a 2024, sem restrição de idiomas. Textos sem resumos, estudos duplicados e artigos que apresentem metodologias não definidas foram excluídos. Resultados e Discussões: Os resultados mostraram que a principal característica da assistência da enfermagem é o cuidar, proporcionar o cuidado adequado das lesões promove cicatrização precoce e melhora a qualidade e a aparência das cicatrizes em longo prazo e o controle álgico, através do uso de opioides, anti-inflamatórios, anticonvulsivantes, antidepressivos, cetamina, benzodiazepínicos, lidocaína e agonistas alfa-2. Estabelecer uma relação de confiança entre o paciente e a equipe de saúde também é fundamental para garantir uma abordagem holística no cuidado ao paciente queimado, reconhecendo suas necessidades individuais e fornecendo suporte psicossocial adequado (Miller et al., 2019). Considerações Finais: A assistência ao paciente vítima de queimadura requer preparo da equipe de enfermagem, em especial do enfermeiro, o qual é responsável por especificar as necessidades do paciente, elaborar o plano de cuidados, supervisionar a execução deste e avaliar a efetividade dos cuidados prestados pela equipe.Palavras-chave: assistência de enfermagem; queimaduras; unidade de queimados.
