Treatment of Bolivian Mucosal Leishmaniasis with Miltefosine

Abstract: In this unrandomized trial, oral miltefosine was at least as effective as heroic doses of parenteral amphotericin B. The cure rate for miltefosine was approximately equivalent to historical cure rates using parenteral pentavalent antimony for mild and extensive disease in neighboring Peru. Although gastrointestinal side reactions do occur with miltefosine, its toxicity profile is superior to that of antimony and far superior to that of amphotericin B--in part because of the inherent attractiveness of oral vers… Show more

“…Considering that MF's efficacy against cutaneous leishmaniasis due to Leishmania (Viannia) species varies from 53% to 91%, 3,4,17,18 and in Brazil, MF was effective against L. (V.) braziliensis in 75% of patients, 1 it would be interesting to investigate whether clinical isolates from unresponsive patients presented low susceptibility to MF in vitro.…”

“…69 Another drug that has been tested for its efficacy in treating CL in Bolivia is miltefosine, with reported cure rates of 88-91% and 58-83% for LCL and ML, respectively. 70,71 Several anecdotal reports describe the use of folk medicine to treat the leishmaniases in Bolivian communities, 72 and many local Bolivia plants have been found to have anti-leishmanial activity in vitro [72][73][74] and in vivo . 74 However, to date, there has been no controlled study to establish whether these plants have a potential application in clinical medicine.…”

“…However, because of considerably more serious side effects and related costs, amphotericin B has been considered as an alternative treatment [18,19]. However, topical application of drugs induces a more rapid healing due to a high drug concentration locally in the lesion and avoids the adverse effects of systemic therapies [12,20].…”
