Treatment of Bleeding Pseudoaneurysms in Patients with Chronic Pancreatitis

Abstract: All hemodynamically stable patients with chronic pancreatitis and bleeding pseudoaneurysms should undergo prompt initial angiographic evaluation and embolization if possible. Repeated angioembolization is feasible in patients with recurrent bleeding, whether initially embolized or operated. Patients with unsuccessful embolization should undergo emergency hemostatic surgery with ligation of the bleeding vessel in the head of the pancreas and distal resection in patients bleeding from the splenic artery or its b… Show more

“…However, other authors recommend early surgical intervention following the AER to avoid re-bleeding (1,2,12). In our case, re-bleeding occurred a few days after the AE and required a second embolization while waiting for the surgery to be scheduled.…”

“…The AE and the surgery are currently the most used therapeutic strategies, such as we performed in our case, and which are considered to be complementary treatments. The AE is recommended as an initial approach in hemodynamically stable patients and represents the treatment of choice in 80% of cases (11,12).…”

“…Частота возникновения ЛА при ОНП составляет 1,2-14 % [1][2][3][4][5][6][7]. Зарегистрированная заболеваемость ХП, осложненного кровотечением из псевдоаневризм, колеблется от 4 до 10 % [8,9]. Наиболее часто развива-ются псевдоаневризмы селезеночной артерии, реже -гастродуоденальной, панкреатодуоденальной и пече-ночных артерий [10].…”

“…Most patients suffer from chronic pancreatitis so the potential perioperative complications and postoperative pancreatic insufficiency should be noted. Most surgical series have documented success rates of 70% to 85% with mortality rates of 20% to 25% and rebleeding rates of 0% to 5% [31][32][33][34]. The treatment options are summarized in Table 4.…”
