Treatment for secondary hyperparathyroidism focusing on parathyroidectomy

Abstract: Secondary hyperparathyroidism (SHPT) is a major problem for patients with chronic kidney disease and can cause many complications, including osteodystrophy, fractures, and cardiovascular diseases. Treatment for SHPT has changed radically with the advent of calcimimetics; however, parathyroidectomy (PTx) remains one of the most important treatments. For successful PTx, removing all parathyroid glands (PTGs) without complications is essential to prevent persistent or recurrent SHPT. Preoperative imaging studies … Show more

“…The recurrent SHPT may arise from the neck, mediastinum, or autografted forearm. The causative parathyroid glands could be localized using imaging modalities such as US, CT, MIBI scintigraphy, or magnetic resonance imaging (MRI) [ 16 ]. Reversal of severe skeletal deformities is impossible, while better control of SHPT may stop its progression and provide a better quality of life [ 8 ].…”

“…Currently, ultrasonography (US), computed tomography (CT), and single-photon emission computed tomography (SPECT)-CT with sestamibi Tc99m can achieve 100% accuracy in the preoperative localization of parathyroid glands ( 2 , 30 ). Therefore intraoperative exploration of parathyroid glands that are not localized is common.…”

“…Parathyroidectomy (PTX) is an effective treatment for drug-refractory primary hyperparathyroidism (PHPT) and secondary hyperparathyroidism (SHPT) ( 1 , 2 ). PHPT and SHPT have a high prevalence in China, and the incidence of PHPT and SHPT is increasing every year ( 3 , 4 ).…”

“…8 , 9 Secondary hyperparathyroidism (SHPT) symptoms include joint discomfort, kidney stones and weakening bones. Secondary hyperparathyroidism is prone to cardiovascular hazards, such as cardiac and cerebrovascular accidents, vascular calcification and other problems 10 ; secondly, it can cause severe generalised itching of the skin, and if calcification occurs, the situation may be even more serious, and even skin ulcers and necrosis 11 ; and there is also the possibility that it may lead to bone damage, including bone pain, decreased bone density, fibrous osteitis, etc. pathological fractures and bone deformities, which will undoubtedly cause a greater impact on the patient's quality of life and working ability.…”
