Introduction: Exercise is emerging as an important part of the treatment in systemic sclerosis-patients with no-mild lung involvement. However, it has not been studied how patients experience physical activity and exercises. We thus explored and described experiences of physical activity/exercises in systemic sclerosis-individuals.Method: Sixteen systemic sclerosis-patients were purposefully recruited to represent variation in gender, age, and lung disease. Semi-structured individual interviews were performed, transcribed and analyzed with qualitative content analysis.Results: Three themes (categories) emerged: 1) Essential for life and health (An effective treatment, Reduces fear of deterioration, and Feeling healthy and satisfied with oneself); 2) Disease-related hinders and other barriers (Disease consequences, Risk of worsening, and Non-disease related barriers); and 3) Self-care and/or support (Self-confidence in physical activity/exercise, and, Education and support from healthcare and other).Conclusions: Physical activity/exercises were experienced as essential for life and health and as an effective treatment. It reduced fear of deterioration and made the participants feel healthy and satisfied with oneself. However, participants also experienced disease-related barriers like shortness of breath and pain, and they expressed a risk of worsening. Participants felt confidence in their physical activity/exercises and expressed that education and support from healthcare could be facilitating. Our findings add new knowledge about how systemic sclerosis-patients perceive physical activity/exercise and can contribute to the development of patient education and PA/exercise programs. A systematic literature review of existing tools used to assess medication adherence in connective tissue diseases: the state of the art for the future development of co-designed measurement tools

Lack of medication adherence is frequent in chronic connective tissue diseases and is associated with poorer health outcomes, low quality of life and economic loss. This research is based on a systematic literature search and aims to identify the surveys and tools used for the assessment of medication adherence in patients with connective tissue diseases (CTDs) and in particular the tools co-designed with patients. A systematic literature review was performed in PubMed and Embase databases searching for studies concerning the application of surveys or tools designed for medication adherence assessment. A specific analysis was also performed to identify which of these existing tools were developed in co-design with patients affected by CTDs. 1958 references were identified, and 31 studies were finally included. Systemic lupus erythematosus was the most investigated disease, followed by the Behçet’s disease. The tools used to assess adherence in CTDs were, in most cases, valid and useful. However, the results showed a certain degree of heterogeneity among the studies and the medication adherence assessment and measurement tools adopted, which were mostly based on selfreported questionnaire. No co-designed tools with patients were found. Low- and non-adherence were explored in some CTDs with valid and useful tools, while other CTDs still need to be assessed. Therefore, more efforts should be made to better understand the specific reasons for the low- and non-adherence in CTDs patients.

Введение. Внутренняя картина болезни (ВКБ) пациента рассматривается и в качестве одного из результатов процесса его адаптации к заболеванию, и в качестве инструмента саморегуляции личности в условиях болезни. Данный психологический конструкт активно изучается современной клинической психологией, что дополняет имеющиеся на сегодняшний день данные, полученные в ходе определения психологических факторов лечебного процесса. Применение многоуровневой модели ВКБ, фокусирующей свое внимание на восприятии болезни и совладании с ней, осуществляется впервые на выборке пациентов ревматологического профиля. Методы. Было обследовано 80 пациентов с ревматическими болезнями в возрасте от 18 до 52 лет: 40 пациентов с ревматоидным артритом (РА), 40 пациентов с системной красной волчанкой (СКВ). Применялся пакет психодиагностических методик, позволяющий описать структуру и содержание различных уровней внутренней картины болезни: опросник выраженности психопатологической симптоматики (SCL 90-r); краткий опросник восприятия болезни; методика для психологической диагностики способов совладания со стрессовыми и проблемными для личности ситуациями. Результаты. В группах сравнения были получены статистически значимые различия: по шкалам восприятия болезни – «понимание болезни», «течение болезни», «контроль болезни», «контроль лечения»; по шкалам копинг-поведения – «поиск социальной поддержки», «принятие ответственности»; по шкалам «соматизация», «депрессивность», «тревога». Установленные корреляции указывают на соотношение между собой параметров различных уровней внутренней картины болезни пациентов в обеих группах. Обсуждение результатов. Параметры внутренней картины болезни пациентов с системной красной волчанкой говорят о стремлении пациентов осознать и преодолеть выраженную неопределенность и непредсказуемость имеющегося у них заболевания, однако это не снижает их эмоциональной вовлеченности в проблему. Внутреннюю картину болезни пациентов с ревматоидным артритом характеризуют худшее понимание и контроль заболевания, а также сфокусированные на эмоциях копинг-стратегии, направленные на дистанцирование от тревожных переживаний, связанных с болезнью.
