Bone and Metabolic Markers in Women With Recurrent Calcium Stones

PurposeThe target of our work was to study several biochemical parameters in phospho-calcic and bone metabolism in blood and urine and the bone mineral density of women with recurrent calcium nephrolithiasis.Materials and MethodsWe conducted a cross-sectional study with a control group of 85 women divided into 3 groups: group 1 consisted of 25 women without a history of nephrolithiasis, group 2 consisted of 35 women with only one episode of calcium nephrolithiasis, and group 3 consisted of 25 women with a history of recurrent calcium nephrolithiasis. Blood and urine biochemical study was performed, including markers related to lithiasis, and a bone mineral density study was done by use of bone densitometry.ResultsPatients in group 3 showed statistically significantly elevated calciuria (15.4 mg/dL), fasting calcium/creatinine ratio (0.14), and 24-hour calcium/creatinine ratio (0.21) compared with groups 1 and 2. Moreover, this group of women with recurrent calcium nephrolithiasis had significantly elevated values of beta-crosslaps, a bone resorption marker, compared with groups 1 and 2 (p=0.000) and showed more bone mineral density loss than did these groups.ConclusionsRecurrent calcium nephrolithiasis in women has a significant association with bone mineral density loss and with values of calciuria, both fasting and 24-hour.

RESUMEN Antecedentes y objetivos: El aumento del consumo de bifosfonatos en la sociedad actual, puede incrementar el riesgo de osteonecrosis mandibular. Se realizó este estudio para valorar si tras la extracción dentaria, después de la administración subcutánea de ácido zoledrónico 7,5µg/Kg o 35 µg/Kg (Zometa®) en ratas Wistar, aparecen signos clínicos, radiográficos e histopatológicos de osteonecrosis y/o inflamación. Lugar de realización: Centro Experimentación Animal del Hospital de Defensa. Material y Métodos: Estudio experimental, in vitro, randomizado, intervencionista. Se utilizaron un total de 30 ratas Wistar (adultas, sanas), repartiéndolas en tres grupos de 10 animales, según sexo, grupo y fármaco: G 0 : Sin tratamiento con ácido zoledrónico. G 1 : Con tratamiento de ácido zoledrónico 7,5µg/Kg subcutáneo una dosis en los días 1, 15 y 30. G 2 : Con tratamiento de ácido zoledrónico 35µg/Kg subcutáneo una dosis en los días 1, 15 y 30. En todos los grupos se realizó exodoncia del primer molar inferior derecho el día 30, sacrificando los animales a las cuatro semanas postextracción, observando clínica, histológica y radiográficamente la aparición de osteonecrosis e inflamación. Resultados: Clínicamente se observaron en un 26,6% falta de epitelización compatible con signos precoces de osteonecrosis mandibular, según criterios de la American Association of Oral Maxillofacial Súrgeons (AAOMS). Esta es dosis dependiente en 3 animales de G 1 (10%) y 5 animales de G 2 (16,6%). Los resultados presentaron significación estadística p<0,011. Ausencia histológica y radiológica de osteonecrosis p< 0,001 e inflamación p <0,001 en todos los grupos. Conclusiones: La administración subcutánea de 7,5µg/Kg o 35µg/Kg de ácido zoledrónico durante cuatro semanas, tras la realización de una extracción dentaria, no da lugar a signos histopatológicos de osteonecrosis e inflamación (p<0,001) pero si a alteraciones clínicas dosis dependientes (p<0,011) compatibles con estadios iniciales de osteonecrosis mandibular según criterios de la AAOMS.

Calcium renal lithiasis: metabolic diagnosis and medical treatment

Calcium renal lithiasis is a frequent condition that affects the worldwide population and has a high recurrence rate. Different metabolic changes may trigger the onset of calcium stone disorders, such as hypercalciuria, hyperoxaluria, hyperuricosuria, hypocitraturia and others. There are also other very prevalent disorders that are associated with calcium calculi, such as arterial hypertension, obesity and loss of bone mineral density. A correct diagnosis needs to be obtained through examining the serum and urinary parameters of mineral metabolism in order to carry out adequate prevention and treatment of this condition. Once the metabolic diagnosis is known, it is possible to establish dietary and pharmacological treatment that may enable monitoring of the disease and prevent recurrence of stone formation. Some advances in treating this pathological condition have been made, and these include use of sodium alendronate in patients with calcium renal lithiasis and osteopenia/osteoporosis, or use of a combination of a thiazide with a bisphosphonate. In summary, calcium renal lithiasis often requires multidrug treatment with strict control and follow-up of patients. RESUMOLitíase renal cálcica é uma doença comum que afeta a população no mundo todo e tem alta taxa de recorrência. Diferentes alterações metabólicas podem desencadear o aparecimento de distúrbios de pedras de cálcio, como hipercalciúria, hiperoxalúria, hiperuricosúria, hipocitratúria e outros. Existem também doenças altamente prevalentes associadas à doença de cálculo de cálcio, como hipertensão, obesidade e perda de densidade óssea mineral. Para realizar prevenção e tratamento adequados, é necessário diagnóstico correto, examinando o metabolismo mineral sérico e urinário. Depois de conhecer o diagnóstico metabólico, é possível estabelecer um tratamento dietético e farmacológico que permita controlar a doença e prevenir a recorrência de cálculos biliares. Há alguns avanços no tratamento dessa doença e incluem o uso de alendronato de sódio em pacientes com nefrolitíase de cálcio e osteopenia/osteoporose, ou a combinação de um tiazídico com um bifosfonato.
