“…The use of fibrinolysis has been extensively studied for various complex pleural processes including hemothorax, for which it has been shown to facilitate drainage and lung expansion, and even to decrease the frequency of surgical intervention [27]. The successful instillation of fibrinolytic agents as a treatment for postoperative coagulated hemothorax in nine patients after cardiac surgery was first reported in 2001 using intrapleural infusion of streptokinase at a standard dose of 250,000 IU in adult patients and 12,000 U/kg in pediatric cases [18]. In 2005, repeated instillation of streptokinase was performed in a patient with a coagulated hemothorax after pleurectomy; twenty‐five billion units of streptokinase in 100 mL of saline solution was administered via the chest tube into the pleural space over 3 days, and resulted in effective drainage of the clotted hemothorax and a rapid recovery without complications [20].…”
