Trapezoidal Achilles Tendon Allograft Plug for Revision Quadriceps Tendon Repair With a Large Tendon Defect

Abstract: Revision quadriceps tendon repair presents a challenging problem for the treating surgeon because of associated anatomic defects such as large tendon-gap deficits and preexistent poor tissue quality. Current methods for revision quadriceps tendon repair use tendon autograft, which may predispose to additional morbidity because the repair relies only on soft tissue fixation. In this Technical Note, we describe a technique for revision of a failed quadriceps tendon repair with a large tendon gap using a trapezoi… Show more

“…Additionally, tendon-to-tendon healing was observed in the present case, which makes it difficult to compare with transpatellar techniques, including some complications (patellar stress fractures and intraarticular damage). [4,6] Regarding autografts, post-traumatic adhesions and poor tissue quality may preclude the harvest of optimal autograft. Also, the autografts have been used combined with transpatellar techniques, which require an optimal tunnel position and tunnel size matched to the autograft.…”

“…[ 3 ] Both repair and healing of quadriceps tears become more challenging in chronic conditions due to the massive retraction of the proximal part of rupture, which may obviate the need for tendon reconstructions. [ 4 ] Gap-filling techniques have better outcomes than primary repair in these cases. [ 5 ]…”

“…Unfortunately, inadequate allograft preparation may affect the restoration of the knee biomechanics despite the important Achilles Allograft resistance. 18 , 19 , 20 , 21 The critical problems during the preparation relate to mismatched dimensions; incorrect thawed, prophylaxis; bone marks and cuts; excessive bone removal; fracture of the bone block by dilation; or allograft contamination. However, support for the important use advantages of the Achilles allograft is given in Table 2 , advantages that include a standardized preparation ( Table 1 ) that helps provide adequate dimension of the allograft, 4 easy manipulation in the operative room, fast procedure, nonmorbidity by the donor site, high strength, healing advantages, multiple-ligament reconstruction capacity, potential to fill bone defects, 14 a high rate of a successful return to sports, 3 and diminished chance of graft tunnel mismatch.…”

“…Tendon defect is one of the common clinical diseases (Hayashida et al, 2018 ; Kunze et al, 2019 ). After the tendon injury, it often leads to limb dysfunction, severe cases, or even disability without repairation in time (Ando et al, 2019 ).…”
