Transvenous Embolization of Dural Carotid Cavernous Fistulas: A Series of 44 Consecutive Patients

Abstract: BACKGROUND AND PURPOSE: Endovascular TVE for DCCF is used for curative purposes, but serious complications can be caused with inadequate embolization. Our aim was to report clinical characteristics, angiographic findings, and results of endovascular TVE in patients presenting with DCCF.

“…5,17,42,52,88 This is demonstrated both clinically by reversal of the signs and symptoms of the CCF and angiographically by fistula obliteration with reversal of retrograde cortical venous flow and vascular steal. In a series of more than 200 patients presenting with posttraumatic direct CCFs, Higashida et al 26 reported complete fistula occlusion with preservation of the ICA in 88% of patients.…”

“…Permanent complications have been reported in 4%-7% of cases. [53][54][55] Despite these risks, TVE can be a safe and effective procedure for many DAVFs and can be used as an adjunct to TAE for a complete cure.…”

“…53 The rates of complete angiographic fistula ablation by TVE have been reported at 71%-87.5%. [53][54][55] The risks of TVE include vessel perforation, infarction, intracranial hemorrhage, and transient or permanent neurologic deficits related to changes in venous drainage course. 55,56 Transient ophthalmoplegia has been reported in 14% of cavernous sinus embolizations, 55 but patients typically have a full recovery.…”

“…Rates of complete fistula obliteration by this technique are high, with reported ranges of 71% to 87%. [68][69][70][71] Specific risks include venous infarction and dural sinus perforation. 1,66 DAVF involving the cavernous sinus are particularly amenable to transvenous embolization, with high rates of successful lesion closure.…”
