Transurethral Bladder Tumor Resection: Intraoperative and Postoperative Complications in a Residency Setting

Abstract: TURBT is a reasonably safe procedure when performed by urologists in training under direct attending supervision. The complication rate was 5.8%, however only 1 case required surgical intervention. Contrary to expected findings, more senior residents were involved in the complications, likely secondary to their disproportionate roles in more difficult resections.

“…In order to close this gap, only in 2012 did an EAU panel attempt to provide recommendations. [20] While the impact of tutors on resident surgeons has been shown, [19] the "reflective techniques", i.e. metacognitive processes that create a greater understanding of the self in performing a procedure, was not used for seniors.…”

“…Conversion to open surgery due to bleeding or perforation is rare [1] but associated with increased surgical trauma and risk of implantation metastasis [2]. Herein we report a case where perioperative angiographic treatment stopped bleeding from a vascular ectasia originating from the internal iliac artery during TURB.…”

“…The complication rate ranges from 5%-43%; however, the majority of studies have reported values less than 15%. Neider et al [46] reported a 2.3% incidence of hematuria requiring transfusion in a residency setting. Additionally, the authors found the rate of bladder perforation to be 3.5%.…”
