Aim:The aim of this study was to estimate the salivary cortisol levels in patients with oral lichen planus (OLP) and recurrent aphthous stomatitis (RAS) and to correlate it with their psychological profile using the Hospital Anxiety and Depression (HAD) scale. Methods: The study population comprised of 60 patients -20 with OLP, 20 with RAS and 20 healthy controls. Salivary cortisol levels were assessed using the chemiluminescent immuno assay technique. Anxiety and depression levels were evaluated using the HAD scale. Results: The Kruskal Wallis mean ranks of salivary cortisol level in the OLP and RAS groups were 33.45 and 33.48 respectively and were higher than the mean rank of the control group with 24.58. But this difference was not statistically significant (P = 0.178). The mean ranks of anxiety score in the OLP and RAS groups were 36.48 and 39.50. The mean ranks of depression scores were 41.88 and 35.25 in the OLP and RAS groups respectively. The control group had a statistically significant lower mean ranks for anxiety and depression (P < 0.001). Anxiety and depression scores showed strong positive correlation in both patient groups. Conclusion: The results suggest that anxiety and depression play a role in the pathogenesis of OLP and RAS. The role of salivary cortisol as a biomarker of stress remains to be validated. Patients suffering from psychosomatic diseases such as OLP and RAS need a comprehensive and holistic treatment approach to manage effectively the psychosocial factors involved in the disease process. Lubricin is a synovial glycoprotein that contributes to joint lubrication. We propose the hypothesis that lubricin is a key modulator of the psychoneuroendocrine-osteoimmune interactome, with important clinical relevance for osteoarthritic pathologies. We consider a variety of neuroendocrine-immune factors, including inflammatory cytokines and chemokines that may contribute to the modulation of lubricin in rheumatic complications. Based on our preliminary immunocytochemistry and fractal analysis data, and in the context of translational research of modern healthcare, we propose that molecular lubricin gene expression modification by means of the novel CRISPR/Cas system be considered for osteoarthritic therapies.

“…More importantly, however, the Center endeavors to ensure that patients are considered from the viewpoint of their physiological totality, meaning that biological variables must be considered in psychiatric patients, immunological diseases and disorders must not be isolated from the psychoneuroendocrine milieu that modulates all aspects of immune regulation, oral and dental conditions and diseases must be recognized for their significant impact on generalized health and disease, and psycho-physiopathology has a profound impact on oral pathology. We recently discussed these complex interactions in the specific context of the role of psychoneuroendocrineimmune interactions in the immune reconstitution inflammatory syndrome (IRIS) in patients with HIV/AIDS treated with antiretroviral therapy [4], as well as psychoneuroendocrine responses to stimuli (e.g., stress) that modulate immune systemic regulation and immune surveillance of the oral cavity [5]. We stand at the edge of a new frontier in health care. The Affordable Care Act-2010 has encouraged researchers and clinicians to work constructively and cooperatively toward maximizing convergent translational patient-centered, effectivenessfocused and evidence-based health care. While developing innovative molecular and cellular biology techniques to inform translational research, the field has validated the sophisticated comparative effectiveness research protocol in translational effectiveness aimed at defining and characterizing the best evidence base for translational convergent health care in general, and translational dental practice in particular. The salient principles and criteria for translational convergent biomedicine of the XXI century are discussed in this paper.
