Background: Chronic liver disease questionnaire (CLDQ) is an established disease specific tool to assess the health related quality of life (HRQOL) in patients with CLD irrespective of the etiology and severity. The objective of the study was to develop the Hindi language version of CLDQ and to assess the validity of the Hindi CLDQ. Methods: CLDQ was adapted into Hindi language by following a standard method of forward and back-translation by two bilingual translators, revision by expert panel and formulation of a consensual version followed by pilot testing and appropriate modifications before final administration. Hindi CLDQ along with short form 36 (SF-36) were administered in 494 patients with chronic liver disease (CLD) and 103 healthy controls (HC). To assess reproducibility, Hindi CLDQ was re-administered in a subsample of 20 patients. Internal consistency was assessed by Cronbach's alpha coefficient. Structural validity was assessed by exploratory factor analysis and construct validity was assessed with help of correlation of SF-36 scores and CLDQ pattern across disease severity groups. Results: The Cronbach's alpha coefficient was 0.93 and intra class coefficient correlation was 0.88 for the average CLDQ score, suggestive of good reliability. An inverse relationship of the average CLDQ score with the disease severity (CLDQ score of Child A vs. B vs. C = 5.2±1.10 vs. 4.73±1.24 vs. 4.15±1.19 respectively; p<0.001) and good correlation with SF-36 scores (average CLDQ score with PCS & MCS; r=0.63 & 0.55; p<0.001) was suggestive of good validity of Hindi CLDQ. The factor analysis identified 6 domains accounting for 62% of variability. The derived cut-off of 6 for the average CLDQ had the AUC of 0.826 with a sensitivity of 82.6 and a specificity of 76.7. Conclusion: Hindi version of CLDQ is found to be a reliable and a valid tool comparable to the original English version. The HRQOL of patients with CLD is lower than healthy controls and HRQOL decreases with increase in disease severity.

Resumo Contexto: O uso disfuncional de álcool é um problema com amplas consequências. Além disso, os efeitos do seu uso sobre a qualidade de vida não foram ainda muito estudados. Objetivo: Avaliar a qualidade de vida de hepatopatas e verificar suas possíveis associações com o consumo de alcoólicos e variáveis sociodemográficas. Método: Estudo transversal realizado com pacientes atendidos em ambulatório de hepatologia, entre abril e dezembro de 2009, avaliando homens entre 20 e 59 anos e escolaridade superior à 5 a série do ensino fundamental. Instrumentos: CAGE, AUDIT, WHOQOL-Bref e questionário sociodemográfico. Resultados: Pacientes CAGE positivos tiveram médias mais baixas no domínio físico do WHOQOL (p = 0,027). Aqueles que nunca usaram alcoólicos tiveram médias mais altas no domínio social comparados aos consumidores de médio e alto risco ou provável dependência no ano (p-valores, respectivamente, 0,023 e 0,036). Abstêmios e usuários de baixo risco registraram médias mais elevadas no domínio social comparados aos usuários de médio risco no último ano (p = 0,031). Conclusão: Os mais baixos níveis de qualidade de vida nos domínios físico (CAGE) e social (AUDIT) registrados pelos pacientes que informaram consumo de alcoólicos de alto risco ou provável dependência são consistentes com achados da literatura. Objective: To evaluate the quality of life of patients with liver diseases and to verify its possible associations with alcohol consumption and sociodemographic variables. Method: Transverse study with masculine patients between 20 and 59 years of age and with at least 5 years of primary school completed, assessed between April and December 2009 in a hepatology service. Instruments: CAGE, AUDIT, WHOQOL-Bref and sociodemographic questionnaire. Results: CAGE positive patients had a lower average in the physical domain of WHOQOL (p = 0.027). Those who never used alcohol had higher scores in the social domain compared to patients with medium and high risk consumption or probable dependence in the past year (p = 0.023 and p = 0.036 respectively). Abstainers and low-risk users recorded the highest average in the social domain compared to medium risk users in the past year (p = 0.031). Discussion: The lowest levels of quality of life in the physical (CAGE) and social (AUDIT) domains reported by patients with alcohol consumption of high risk or probable dependence are consistent with findings in the literature.

Background and Aim Assessment of health‐related quality‐of‐life (HRQOL) in patients with chronic liver disease (CLD) requires the use of validated instruments that are understood by patients in their native language. We previously translated the Chronic Liver Disease Questionnaire into the Singapore‐Mandarin version (CLDQ‐SG). This study aims to examine the internal consistency and validity of the CLDQ‐SG in patients with CLD. Methods We conducted a cross‐sectional study of adult patients with CLD seen in a tertiary center in Singapore who completed both the CLDQ‐SG and Short Form Health Survey 36 version 2 (SF‐36v2) questionnaires. Internal consistency of the CLDQ‐SG was assessed using Cronbach's alpha coefficient. Convergent and divergent validity of the SF‐36v2 was assessed using the Spearman correlation coefficient, while discriminant validity was assessed using the Jonckheere‐Terpstra test for trend. Exploratory factor analysis was performed to evaluate the factor structure of the CLDQ‐SG. Results We enrolled 242 subjects (68.2% males, median age 67 years). Predominant etiology of CLD was chronic hepatitis B. Severity of CLD was divided into noncirrhotic (67.3%), compensated cirrhosis (24.0%), and decompensated cirrhosis (8.7%). Item convergent and discriminant validity of the CLDQ‐SG was excellent, with 100% scaling success in all six domains. All domains exhibited good internal consistency, with Cronbach's α > 0.70. We observed a consistent trend of a reduction in mean CLDQ‐SG score in the three groups reflecting the discriminant validity of the CLDQ‐SG to assess changes in HRQOL in different severities of CLD. Factor analysis of the CLDQ‐SG demonstrated an independent factor assessing sleep. Conclusion The Singapore‐Mandarin version of CLDQ‐SG is a valid and reliable instrument to measure HRQOL in patients with CLD.
