BackgroundFew studies have assessed whether the amelioration of the clinical signs of polycystic ovary syndrome (PCOS) achieved by treatment leads to improvement in the health-related quality of life (HRQoL) of patients. This study was aimed to examine the HRQoL of ethnic Chinese women with PCOS who received metformin treatment.MethodsThis prospective study was conducted at a medical center in Taiwan. Study participants aged 18–45 years were diagnosed as having PCOS according to the Rotterdam criteria, and all received metformin treatment. Their HRQoL was assessed using generic (WHOQOL-Bref) and PCOS-specific (Chi-PCOSQ) instruments. Mixed effect models were used to examine the effects of metformin on repeatedly measured HRQoL. Additional analyses using stratified patients characteristics (overweight vs. normal; hyperandrogenism vs. non-hyperandrogenism) were done.ResultsWe recruited 109 participants (56 % were overweight, 80 % had hyperandrogenism). Among the domain scores of WHOQOL-Bref, the psychological domain score was the lowest one (12.64 ± 2.2, range 4–20). Weight (3.25 ± 1.59, range 1–7) and infertility (3.38 ± 1.93, range 1–7) domain scores were relatively low among the domain scores of Chi-PCOSQ. Overweight and hyperandrogenic patients had significantly lower HRQoL as compared with those of normal weight and non-hyperandrogenic patients, respectively. Metformin significantly improved the physical domain of WHOQOL-Bref (p = 0.01), and the infertility (p = 0.043) and acne and hair loss aspects (p = 0.008) of PCOS-specific HRQoL. In the subgroup analysis, significantly improved HRQoL following metformin treatment appeared for only overweight and hyperandrogenism subgroups.ConclusionsMetformin might improve health-related quality of life of polycystic ovary syndrome women by ameliorating psychological disturbances due to acne, hair loss and infertility problems, especially for overweight and hyperandrogenic patients.

Aims and objectives: To translate and validate a Taiwanese version of the Self-Efficacy for Appropriate Medication Use Scale (SEAMS). Background: The major barrier in the management of atrial fibrillation is the lack of selfefficacy to medications adherence. Patients' knowledge of the nature of atrial fibrillation and cardiovascular, cerebrovascular risk factors and anticoagulants is a significant factor affecting individuals' adherence. However, few instruments have been developed to assess patients' self-efficacy to medications adherence and none has been translated into Taiwanese. Design: This study used a standard ''forward-backward'' procedure, which was used to translate SEAMS into Taiwanese language. Reliability was tested for internal consistency. Validity was confirmed using criterion-related validity. Methods: Data collection for this research occurred from October 13, 2015 to October 26, 2016. The sample included atrial fibrillation outpatient. Participants simultaneously completed the Taiwanese version of the Self-Efficacy for Appropriate Medication Use Scale (SEAMS-T). A total of 151 individuals completed the questionnaire. Results: Coefficient alpha was 0.931 for atrial fibrillation patients. Conclusions: Findings provide support for the validity of the Taiwanese version of the Self-Efficacy for Appropriate Medication Use Scale (SEAMS-T). Relevance to clinical practice: The translation and validation of an instrument evaluating the self-efficacy for medication adherence in atrial fibrillation contribute to assessing the provided educational intervention.

ВведениеРешение вопросов недостаточной приверженности к терапии является одной из актуальных проблем со-временной медицины и общества, и в основном ассо-циируется с длительной терапией хронических забо-леваний. Низкая приверженность к лечению является одной из главных причин уменьшения выраженности терапевтического эффекта, существенного повыше-ния вероятности развития осложнений основного за-болевания, приводящего к снижению качества жизни больных и увеличению затрат на лечение.Вопросы оценки и прогнозирования приверженности к лечению у конкретного больного, наряду с поиском наиболее эффективных методов для ее повышения, не потеряли своей актуальности. До сих пор «золотого стан-дарта» определения приверженности пациента к терапии не существует. Среди десятка методов диагностики и контроля того, как больной соблюдает данные ему ре-комендации по приему лекарственных препаратов и из-менению образа жизни, нет ни одного, лишенного не-достатков и с высокой точностью дающего ответы на дан-ные вопросы.Одним из наиболее часто используемых методов оценки приверженности является анкетирование па-циентов. Согласно одной из классификаций методов оценки приверженности опросники и шкалы выделяются в отдельную группу наряду с группами клинических, фармакологических и физических методов. Кроме того, необходимо учитывать неоднородность понятия «приверженность», например, наличия потенциальной приверженности к лечению, которую диагностируют у пациента до назначения ему терапии, и фактической приверженности, выявляющей реальное соблюдение конкретным больным данных ему врачебных реко-мендаций по приему лекарственных препаратов [1,2]. Тест Мориски-ГринаНаиболее известным, простым и лаконичным при-знан тест Мориски-Грина, валидированный в 1985 г. и опубликованный авторами в 1986 г. С тех пор данный тест широко применяется в клинической практике для скрининга приверженности пациентов к приему ле-карственных препаратов [3].
