Transient ischemic stunning of the myocardium in stress thallium-201 gated SPET myocardial perfusion imaging: segmental analysis of myocardial perfusion, wall motion and wall thickening changes

Abstract: Prolonged and persistent myocardial stunning has recently been demonstrated using technetium-99m sestamibi gated single-photon emission tomography (SPET) myocardial perfusion imaging post exercise or pharmacological stress test. In this study, we investigated the early postischemic transient myocardial stunning on early and delayed poststress thallium-201 gated SPET myocardial perfusion imaging using segmental wall motion (WM) and wall thickening (WT) analysis. A total of 1,680 segments from 84 patients' studi… Show more

“…In 58 patients with previous myocardial infarction Bavelaar-Croon et al [20] found post-stress LVEF of 2% or more lower than the resting LVEF. Very recently, Santiago et al [21] have shown early transient myocardial stunning using 201 Tl re-injection, manifested as post-stress decreased LVEF, wall motion and wall thickening in ischaemic segments and related to the severity of ischaemia.…”

“…This perfusion tracer has known physiological advantages [55], but yields poorer statistics, because of its longer half-life and lower energy. Even though useful gated 201 Tl images have been obtained with modern S-SPECT cameras [56, 57], usage had been limited by image quality. Since 201 Tl acquisition commences almost immediately after stress, improved gating with 201 Tl, using DNM, is expected to result in increased detection of myocardial stunning, which is indicative of severe CAD.…”
