Comparison of exercise and pharmacological stress gated SPECT in detecting transient left ventricular dysfunction

Abstract: Dipyridamole is believed to be less likely than exercise to induce ischemia. However, in this study, TLVD after stress was observed following not only exercise but also pharmacological stress, consistent with ischemia.

“…Further studies are needed to determine associations between albuminuria and scintigraphic parameters in patients with CKD. Previous studies of post-stress myocardial dysfunction focused on patients with exercise-induced ischemia, however, some reports have demonstrated post-ischemic stunning after vasodilator stress [ 23 , 27 29 ]. Myocardial perfusion imaging with adenosine relies on including relative flow heterogeneity in myocardial regions supplied by normal and diseased coronary arteries, and in some patients with significant stenosis, adenosine may induce true ischemia as a result of coronary steal [ 30 ].…”

“…An abnormally high stress-to-rest mean left ventricular volume (SRLVV) ratio is also one of the stress-induced abnormalities in myocardial perfusion imaging (MPI) that may be unrelated to obstructive coronary artery disease [7]. High SRLVV ratio-transient ischemic dilatation (TID)-is a prognostic factor in cardiovascular disease in persons with diabetes mellitus [8] and correlates with difference in stress and rest LV ejection fractions [9], which is a risk factor for future cardiac events [10]. Considering the simultaneous correlation of ED and TID by abnormalities of MPI which may occur in the absence of obstructive coronary artery disease, we speculate ED may associate with TID through microvascular mechanisms [6,7].…”
