Reabilitação cardíaca PRIMEIROS REGISTROS HISTÓRICOSEm 10 de julho de 1893, o Dr. Daniel Hale Williams, um cirurgião de Chicago, operou com sucesso um homem de 24 anos que havia sido esfaqueado no coração durante uma briga. A facada foi ligeiramente à esquerda do esterno e no centro do coração. Inicialmente considerado superficial, o ferimento, com o passar do tempo, apresentava um sangramento persistente, acompanhado por queixas de dor e de sintomas pronunciados de choque. Williams abriu o tórax do paciente e amarrou a artéria e a veia lesionadas dentro da parede torácica, provavelmente causando a perda de sangue. Ele então notou uma ruptura no pericárdio e um ferimento no coração 1 .Como o ferimento no ventrículo direito não estava sangrando, o cirurgião não deu nenhum ponto no ferimento no coração, decidindo fechar o buraco no pericárdio com pontos. O paciente se recuperou e Williams relatou esse caso somente 4 anos depois 1 . O procedimento, frequentemente relatado, é provavelmente a primeira cirurgia bem-sucedida e documentada sobre uma facada no coração. Na época, a cirurgia foi considerada ousada e, embora ele não tenha realmente dado um ponto na ferida no coração, seu tratamento parece ter sido adequado. Alguns anos depois, outros cirurgiões suturaram feridas cardíacas, mas os pacientes não sobreviveram. Ludwig Rehn, cirurgião de Frankfurt, Alemanha, realizou o que muitos consideram a primeira operação cardíaca bem-sucedida 2 .Em 1896, um homem de 22 anos foi esfaqueado no coração e desmaiou. A polícia o encontrou pálido, coberto de suor frio e com muita falta de ar. Seu pulso estava irregular e suas roupas estavam encharcadas de sangue. A piora de sua condição, de acordo com as anotações do próprio Dr. Rehn, resultaram em aumento do hemotórax, sendo a ferida aberta de 1,5 cm no ventrículo direito suturada com uma agulha para intestino delgado, com sutura de seda amarrada na diástole. Dez anos após esse procedimento, o Dr. Rehn tratou mais 124 casos, registrando uma mortalidade de 60%, considerada um grande feito naquela época 3 .Primeiro americano a relatar o reparo bem-sucedido de uma ferida cardíaca, o Dr. Luther Hill operou um menino de 13 anos, vítima de múltiplas facadas. O primeiro médico a chegar ao local do acidente encontrou o menino em estado de choque profundo e lembrou que o Dr. Hill havia falado sobre o reparo de feridas cardíacas durante uma reunião da sociedade médica local no Alabama. Chamado para atender essa emergência, o Dr. Hill, acompanhado por mais seis médicos, realizou a cirurgia na mesa da cozinha do paciente, em um barraco decadente. A iluminação era fornecida por duas lamparinas de querosene emprestadas de vizinhos. Um médico administrou anestesia com clorofórmio 4 .

Cardiac surgery has been evolving to include minimally invasive, hybrid and transcatheter techniques. Increasing patient age and medical complexity means that critical care management needs to adapt and evolve. Recent advances have occurred in several areas, including ventilation, haemodynamics and mechanical circulatory support, bleeding and coagulation, acute kidney injury, and neurological management. This narrative review describes standard care, recent advances, and future areas of research in the critical care management of patients undergoing cardiac surgery.

Background:In critical patients, left ventricular ejection fraction and fractional shortening are used to reflect left ventricular systolic function. An emerging technique, two-dimensional-strain echocardiography, allows assessment of the left ventricle systolic longitudinal deformation (global longitudinal strain) and the speed at which this deformation occurs (systolic strain rate). This technique is of increasing use in critical patients in intensive care units and in the peri-operative period where preload constantly varies. Our objective, in this prospective single-center observational study, was to evaluate the effect of fluid resuscitation on two-dimensional-strain echocardiography measurements in preload-dependent critically ill patients. We included 49 patients with preload dependence attested by an increase of at least 10% in the left ventricular outflow track velocity-time integral measured by echocardiography during a passive leg raising maneuver. Echocardiography was performed before fluid resuscitation (echocardiography 1) and after preload independency achievement (echocardiography 2).Results: Two-dimensional-strain echocardiography was feasible in 40 (82%) among the 49 patients. With preload dependence correction, the absolute value of global longitudinal strain and systolic strain rate was significantly increased from, respectively, − 13.3 ± 3.5 to − 18.4% ± 4.5 (p < 0.01) and − 1.11 s −1 ± 0.29 to − 1.55 s −1 ± 0.55 (p < 0.001). The fluid resuscitation affects GLS and SSR in preload-dependent patients, with a shift, for GLS, from pathological to normal values. Conclusion:In critically ill patients, the assessment of the systolic function by two-dimensional-strain echocardiography needs prior evaluation of preload dependency, in order to adequately interpret this variable. Future studies should assess the ability of global longitudinal strain to guide fluid management in the critically ill patients.
