Transepithelial elimination in necrobiosis lipoidica

Abstract: SUMMARY Elimination of necrotic material through the hair follicle as a means of excretion has been observed in three cases of necrobiosis lipoidica. Clinically this process presents as comedo‐like plugs localized mainly in the periphery of the lesions.

“…Disorders illustrating TEE include the following: calcinosis cutis, osteoma perforans, granuloma annulare, elastosis perforans serpinginosa, reactive perforating collagenosis, per¬ forating folliculitis,1 and, recently, necrobiosis lipoidica diabeticorum. 3 This list should be expanded to include cutaneous chromomycosis, and, quite likely, other infectious granulomas. Zaias The precise mechanisms and mes¬ sages responsible for induction of TEE are not known.…”

“…[2] Rarely, transepidermal elimination and lymphoid follicle formation have been seen. [1011] In our series, we found the interstitial granulomatous pattern to be the commonest. A palisading granuloma with a tired appearance which is typically described in NL was seen in only one patient.…”

“…The perforating variant was presented by Parra in 1977, who published 3 clinical cases of transfollicular elimination of degenerated collagen presenting clinically as hyperkeratotic plugs on the surface of the plaques [2]. …”
