EinleitungDie diffuse Proliferation der glatten Muskulatur der Lunge (DSMPL) ist eine selten diagnostizierte Lungenerkrankung un− klarer Ätiologie. Sie ist gekennzeichnet durch eine diffuse Proli− feration der glatten Muskulatur unter Einbeziehung der Wände von Alveolen, Bronchien und pulmonalen Gefäßen. AbstractA 50−year old female forest worker presented with the com− plaints of progressive exercise induced dyspnea and the history of a recent syncope. Right heart catheterisation revealed the diagnosis of severe precapillary pulmonary hypertension. Venti− lation perfusion lung scan indicated the existence of multiple pulmonary emboli with high probability. However, pulmonary angiography did not confirm this finding. Computed tomography of the lung demonstrated a slight mosaic pattern of diffuse ground glass opacities. After video assisted thoracoscopy, the histologi− cal diagnosis of diffuse smooth muscle proliferation of the lung could be concluded from the removed lung tissue. Diffuse smooth muscle proliferation of the lung is a rarely diagnosed disease, however it should be considered in patients with sus− pected pulmonary embolism by ventilation perfusion scan but a normal finding in pulmonary angiography, who are suffering from otherwise not explained pulmonary hypertension. “…Marker proteins described to date, such as ␣-smooth muscle actin, vimentin, and desmin, are not unique to LAM cells, and specific cell-surface markers have not been reported. LAM cells do differ from normal smooth muscle cells by the presence of gp100, a 100-kDa glycoprotein found in melanoma that reacts with a monoclonal antibody, HMB45 (13), but percentages of HMB45-positive LAM cells varied from 17% to 67% in biopsies (2), with an inverse relationship between percentages of cells positive for proliferating cell nuclear antigen and HMB45 positivity (14). We therefore decided on genotype as a criterion for identification of LAM cells.…”

PEComas are a family of neoplastic lesions that share overlapping morphology, immunohistochemistry, and ultrastructure that include angiomyolipoma, lymphangioleiomyomatosis, clear cell ''sugar'' tumor of the lung as well as similar tumors occurring in a variety of visceral, cutaneous and soft tissue sites throughout the body. The defining histopathological features are epithelioid cells with a perivascular distribution containing clear to pale eosinophilic granular cytoplasm and a round-to-oval centrally located nucleus with an inconspicuous nucleolus. Immunohistochemically, coexpression of melanocytic (HMB-45 and/or Melan-A) and myoid markers are characteristic. In the present study, we describe three PEComas occurring in the head and neck (nasal cavity and larynx) and discuss the behavior of these distinctive tumors and review the literature of head and neck PEComas. The importance of recognizing this entity will ensure its consideration in the differential diagnosis of tumors of the head and neck with a similar morphology. The histogenesis of PEComas still remains elusive and additional cases with a prolonged follow up remain important to accurately determine the behavior of these distinctive tumors. Complete surgical excision still remains the treatment of choice for histologically benign PEComas.
