Abstract. Transarterial chemoembolization (TACE) is an established therapeutic approach for the treatment of hepatocellular carcinoma (HCC). Although patients who undergo TACE may have prolonged survival, there are indications that the malignancy of residual HCC tissue can increase subsequent to the procedure. Although hypoxia, which occurs during TACE due to ischemia, is known to contribute to angiogenesis, little is known with regard to the undesirable effects of chemotherapeutic agents on residual HCC cells. Doxorubicin is one of the most commonly used drugs in TACE. The aim of the present study was to analyze alterations in Hep3B and HepG2 human HCC cell lines surviving doxorubicin treatment in vitro. Initially, the toxic concentration range was determined, and doxorubicin was subsequently applied in concentrations that killed >80% of the HCC cells. During the first days subsequent to treatment, surviving cells had higher expression levels of the epithelial-mesenchymal transition marker SNAIL, and exhibited increased migratory activity compared with control cells. At 3 weeks after the first doxorubicin treatment, surviving HCC cells tolerated significantly higher doxorubicin concentrations compared with control cells. As a potential explanation for this doxorubicin resistance, significantly increased mRNA expression levels of ATP-binding cassette ABCB1 (multidrug resistance protein 1) and ABCC1 (multidrug resistance-associated protein 1) were observed by reverse transcription-quantitative polymerase chain reaction. In summary, these findings indicate that, following TACE treatment, hypoxia as well as doxorubicin may induce a more malignant phenotype in surviving HCC cells and decrease susceptibility to further chemotherapeutic treatment.

ВведениеПервичный рак печени занимает 5-е место по распространенности и 2-е по смертности среди он-кологических заболеваний, при этом в 2012 г. заре-гистрировано 782 000 заболевших. В структуре пер-вичных злокачественных опухолей печени гепато-целлюлярный рак (ГЦР) составляет 85-90% [8]. К сожалению, в большинстве наблюдений ГЦР диа-гностируется в поздних стадиях, когда невозможно проведение радикального лечения. Трансартериаль-ная химиоэмболизация (ТАХЭ) печени является ме-тодом выбора при промежуточной стадии ГЦР, ко-торая встречается наиболее часто. Селективность манипуляции позволяет минимизировать системное воздействие на организм и обеспечивает высокую эффективность за счет доставки химиоэмболизиру-ющих агентов непосредственно в сосудистое русло опухоли. Интерес вызывает также наличие широко-го разнообразия режимов выполнения ТАХЭ как в качестве монотерапии, так и в комбинации с други-ми терапевтическими опциями.Цель исследования -оценка результатов лече-ния больных ГЦР после ТАХЭ в качестве самостоя-тельной лечебной опции, «моста» к трансплантации печени и в рамках комбинированной терапии. Материал и методыВ исследовании ретроспективно проанализиро-ваны результаты лечения 29 больных ГЦР, которым Цель -оценить результаты лечения больных ГЦР после ТАХЭ в качестве самостоятельной лечебной опции, «моста» к трансплантации печени и в рамках комбинированной терапии. Материал и методы. Представлен опыт применения транс-артериальной химиоэмболизации (ТАХЭ) в лечении 29 больных гепатоцеллюлярным раком. Лечебные процедуры выпол-нены в рамках монотерапии, «моста» к трансплантации печени и комбинированного лечения. Суммарно было проведено 48 сеансов химиоэмболизации: один и два сеанса получили 44,9% больных, три процедуры -10,2% пациентов. Средний промежуток между сеансами равнялся 76,2±116,2 сут (8-139 сут). Результаты. Постэмболизационный синдром (повы-шение температуры тела, тошнота, боль) наблюдали у 24,1% больных после 1-го этапа, у 50 и 33,3% больных после 2-го и 3-го этапов соответственно. Средний период ожидания трансплантации печени в группе пациентов bridge therapy был равен 8,5±6,8 мес (1-20 мес). Медиана выживаемости после ТАХЭ в группе монотерапии составила 9 мес.
