Transapical NeoChord mitral valve repair

Abstract: Transapical off-pump mitral valve repair (MVr) with NeoChord implantation has become widely applied in Europe for patients presenting with severe mitral regurgitation due to leaflet prolapse or flail. The procedure is performed under real-time 2D-and 3D-transesophageal echocardiography (TEE) for both implantation and neochordae tension adjustment allowing real-time monitoring of hemodynamic recovery. Preoperative anatomic and echocardiographic selection criteria, procedure refinement, as well as innovative ex-… Show more

“…There were 4 patients with type B and 4 patients with type C MR in our series. The rate of re-intervention for trans-apical neochord failure during the follow-up period was (3/24, 12.5%) higher than the value reported by Colli et al (22) (4/49, 8.2%). This may be because the number of type B and C patients in our study were higher than the patients in their study (8/26, 5/49 respectively) (22).…”

“…27,28 In this study, virtual transapical neochordae repair was modeled under optimal implantation conditions, that is, following the manufacturer's recommendations 29 and the clinical step-by-step guide. 26,30 Clinically, three or four neochordae are implanted on average in order to balance the tension per neochorda, provide structural support, and avoid excessive mechanical stress on the leaflets. 4 Thus, three (NC3) and four (NC4) neochordae were modeled in this study for each PML prolapse model, as seen in Figure 1B.…”

“…Since the introduction of the technology, more than 1000 patients in Europe and the USA have successfully undergone the NeoChord procedure [9]. This system allows minimally invasive off-pump delivery of the ePTFE chordae to the prolapsing or flail MV leaflet and enables the adjustment of the length of the chordae under direct echocardiographic control while the valve is fully functioning under physiologic conditions of the beating heart.…”

“…This system allows minimally invasive off-pump delivery of the ePTFE chordae to the prolapsing or flail MV leaflet and enables the adjustment of the length of the chordae under direct echocardiographic control while the valve is fully functioning under physiologic conditions of the beating heart. This TEE guided procedure is currently standardized and is described in detail in recent publications of Colli et al [5,9] and Samalavicius et al [10].…”
