ObjectivesSickness absence (SA) is a widely studied integrated measure of health status. To better understand how SA behaves over time (SA trajectory) a longitudinal and individual-centred approach will allow identifying groups of individuals who share common characteristics. The aim of this study was to identify and describe SA trajectories and to assess employment conditions and diagnosis groups as determinants.SettingWorking-life and sickness absence administrative records from a representative sample of affiliated with the Spanish Social Security system.Participants38 420 workers affiliated with the Spanish Social Security system, born 1949 to 1969 or 1970 to 1990, resident in Catalonia who had SA between 2012 and 2014 (75 212 episodes).ResultsWe identified three different SA trajectories in both birth cohorts for men and women: low-stable (86.2% to 90.8% of individuals), decreasing (4.4% to 5.9% of individuals) and increasing (4.1% to 8.7% of individuals) accumulated days of SA. The main characteristic of SA trajectories was the medical diagnosis group. The increasing SA trajectory had a higher proportion of workers with SA due to mental disorders compared with the other trajectories. The association analysis showed diagnosis group strongly related with all SA trajectories, particularly SA due to mental disorders showed the strongest association with the increasing trajectory among young men (adjusted OR (aOR): 42.40, 95% CI 17.03 to 105.57). Low salary levels exhibited a strong relationship with decreased accumulation of SA days over time for old women (aOR: 2.08, 95% CI 1.36 to 3.18) and men (aOR: 2.75, 95% CI 1.77 to 4.27). Unskilled manual occupations were associated with increasing trajectories among young women (aOR: 1.36, 95% CI 1.01 to 1.84). No significant differences were observed for other employment conditions across trajectories.ConclusionsWorkers with mental disorders are more likely to have increased days of SA, whereas low salary levels at later ages are related to a decrease in SA days over time. Special attention to preventing the course of mental disorders at young and middle age is warranted.

Work disability may originate early during work history and involve sickness absences (SA) and eventually permanent disability. We studied this process over 15 years. Questionnaire data collected in 1981 on health, working conditions, and lifestyle of Finnish municipal employees aged 44–58 years (n = 6257) were linked with registers on SA (≥10 workdays), disability pension, and death from the period 1986–1995. Trajectory analysis was used to assess development in SA (days/year) over 5 years (1981–1985). We analyzed determinants of the trajectories with multinomial regression, while trajectory membership was used as a predictor of disability pension (DP) during the subsequent 10 years in survival analysis. Three SA trajectories emerged: increasing (women: 6.8%; men: 10.2%), moderate (21.2%; 22.7%), and low. In a mutually adjusted model, the increasing trajectory in women was associated with baseline musculoskeletal (MSD), mental and respiratory disorders, injuries, obesity, sleep problems, and low exercise (effect sizes OR > 2), and in men with MSD, sleep problems, smoking, low exercise, and non-satisfaction with management. The moderate trajectory associated with MSD, ‘other somatic disorders’, sleep problems, and awkward work postures in both genders; in women, also overweight, cardiovascular and respiratory morbidity, and (inversely) knowledge-intensive work, and in men, smoking and mental disorders were thus associated. Ten-year risks of DP contrasting increasing vs. low SA were more than 10-fold in both genders and contrasting moderate vs. low SA 3-fold in women and 2-fold in men. These findings emphasize the need for early identification of workers with short-term problems of work ability and interventions regarding lifestyle, health, and working conditions, to help prevent permanent disability.

Objetivo: Identificar os afastamentos laborais, concedidos por licenças saúde, justificados por adoecimentos e agravos de trabalhadores de secretarias do setor público municipal, no extremo sul do Brasil. Metodologia: Trata-se de uma pesquisa transversal descritiva e analítica, com abordagem quantitativa. A coleta de dados foi extraída de um banco do Setor de Saúde Ocupacional do município do estudo. Na análise prévia dos dados, foi aplicada a estatística descritiva. Por se tratar de dados secundários, foram respeitadas as diretrizes éticas do Conselho Nacional de Saúde. Resultados: Dos 4.120 afastamentos laborais dos servidores municipais, por licença saúde, 52,6% foram de servidores professores; constatou-se que, não obstante os afastamentos laborais, tenham sido justificados por até 6 diagnósticos, codificados segundo a classificação internacional de doenças, 95,6% foram anotados por um diagnóstico; do conjunto de doenças anotadas nas licenças saúde, 68% se enquadraram em 5 grupos da classificação internacional das doenças: “Fatores que influenciam o estado de saúde e o contato com os serviços de saúde”; “Doenças do Sistema Osteomuscular e do Tecido Conjuntivo”; “Doenças do Aparelho Respiratório”; “Doenças do Aparelho Digestivo”; e “Sintomas, Sinais e Achados Anormais de Exames Clínicos e de Laboratório; quando considerado o código da categoria da classificação internacional, o enquadramento do conjunto de doenças abrangeu 367 categorias. Conclusão: Espera-se contribuir para o desenvolvimento de estratégias de gestão que visem a segurança e as condições saudáveis do trabalhador e dos ambientes laborais.
