Totally laparoscopic transhiatal esophago-gastrectomy without thoracic or cervical access: The least invasive surgery for adenocarcinoma of the cardia?

Abstract: In selected cases, it is possible to perform a distal esophagectomy entirely by laparoscopy, without the need for any thoracic or cervical access.
