Total conservative parotidectomy for primary benign pleomorphic adenoma of the parotid gland: A 25‐year experience with 229 patients

Abstract: A 25-year experience with total conservative parotidectomy for primary benign pleomorphic adenoma (PBPA) of the parotid gland in a consecutive series of 229 patients has been reviewed. This study focuses on survival, morbidity, PBPA recurrence, postoperative facial nerve dysfunction, and Frey's syndrome. Surgical death was never encountered in this study. The overall incidence for postoperative hemorrhage, hematoma, seroma, skin necrosis, great auricular nerve schwannoma, and keloid scar was 1.7%, 6.1%, 4.8%, … Show more

“…Retrograde dissection of the facial nerve is more popular in china with encouraging results. 1 Function preserving parotid surgery has relatively low complication rate like the use of a modified facelift incision, preservation of the great auricular nerve and normal parotid parenchyma. Ear sensation returned more rapidly and more completely in patients in whom the posterior branch of the great auricular nerve is preserved than in those in whom the nerve is sacrificed.…”

“…[1][2][3][4] Pleomorphic adenoma is most common in middle age and in women, 5 and although rare in children, it remains the most common solid parotid mass in the pediatric population. [6][7][8][9] These tumors arise directly from salivary gland tissue and consist of epithelial and mesenchymal cells; the latter can give rise to osseous, cartilaginous, hyaline, and myxoid elements, hence the name "mixed tumor."…”

“…For pleomorphic adenoma it is known that local excision/enucleation are accompanied by 20-45% rate of local recurrence (LR) [1,3,5] and that this is due to seeding rather than multicentricity [1,7,8]. some authors have advocated TCP to reduce the incidence of LR while maintaining acceptable morbidity patterns [8,9]. However for most cases SP remains 'an adequate procedure for small tumours confined to superficial lobe [1,3,5,1 0].…”
