Gonadoblastomas are known to develop in dysgenetic gonads, especially so, if Y chromosome material is present. A 20-years- old girl who noticed breast development since the age of 12 years presented with primary amenorhoea, distension of lower abdomen and intermittent pain for two months. She had breakthrough bleeding with six months of estrogen replacement. Tanner breast stage was fi ve and pubic hair stage was also fi ve. Examination revealed a mass in the lower abdomen extending into hypogastrium, umbilical and lumbar regions. Her gonadotropin levels were grossly elevated. Karyotyping showed 46XY. CT scan of abdomen showed a 17X11 cm mass in the pelvis without visible gonads. Surgical excision of the mass along with bilateral salpingophorectomy was performed. Histopathology revealed the mass to be a dysgerminoma, while the right gonad lodged gonadoblastoma. She was diagnosed as a rare case of Swyer syndrome. For the palliative therapy, large number of psoriatic patients in India often flocks around alternative stream of ABSTRACT Background: Several oil based AYUSH formulations are available for the treatment of psoriasis. In the recent time the importance given to Wrightia tinctoria has totally side-lined the quality of oil used in such preparation. Therefore, the importance of oil used in the formulation with reference to rancidity and how that would affect the therapeutic effect of Wrightia tinctoria needs to be studied. Methods: In the present study we have adopted the following methods such as acid value, DPPH assay, nitric oxide inhibition, anti-glycation, CAM assay and fibroblast toxicity. The base oil used was studied for the above then the oil that showed least acid value was incorporated with Wrightia tinctoria extract and then studied by the above methods. Results: The above study clearly show that the activity of Wrightia tinctoria decrease significantly when it was formulated in oil with high rancidity. Whereas the activity of Wrightia tinctoria was well preserved in oil with least rancidity. The contribution of acid value of oil in decreasing the activity of Wrightia tinctoria was much higher than the effect of acid value in increasing DPPH activity, Nitric oxide production, inducing glycation event and angiogenesis and causing fibroblast toxicity. Conclusions: The present study clearly concludes that the acid value of base oil would not only worsen psoriasis but also can inactivate and convert the herbal metabolites to toxic elements. Due diligence is necessary in preparing AYUSH drugs where not only the quality of herbs should be considered but also the oil base as well.
Caratol-e capsules was studied for its effect in altering the stomach ecosystem and found that caratol e capsules does not affect the same. The possible reason for the administration of two capsules in night may be due to its likely effect on melatonin –MSH interaction and this sufficiently explains the importance of caratol e capsules in the treatment of vitiligo and the importance such treatment during night hours as well. Findings of the study discussed in the paper.
Sixty five patients who had undergone surgery for parotid tumours over a period of nine years were studied retrospectively. There were 45 males and 20 females. There were 49 benign and 16 malignant tumours. Age ranged 35-69 years; mean 47 years. Pain (6%) and facial palsy (3%) were unusual features. Size of tumour at presentation varied from 1.5 to 12 em. Superficial parotidectomy was the commonest operation performed and TotaURadicallEnended parotidectomy was done in selected cases. Postoperative radiotherapy,was used in six cases. No recurrences were noted following surgery for benign tumours. Howe"er a local recurrence rate of 37%, was found for malignancies (6/16 cases).
